M蛋白分型联合本周蛋白检测在多发性骨髓瘤中的应用

摘　　要：[目的]探讨血清M蛋白联合尿本周蛋白检测在多发性骨髓瘤中的应用价值。[方法]选择335例多发性骨髓瘤患者,利用毛细管区带电泳-免疫消减法和免疫固定电泳分别对血清M蛋白和尿本周蛋白进行检测,分析两种方法的优势及局限性。[结果]M蛋白分型结果显示,238例为M蛋白阳性,检出率为71.0%,其中Ig G型146例(61.4%),Ig A型55例(23.1%),Ig M型5例(2.1%),轻链型32例(13.4%)。尿本周蛋白结果为,208例患者结果为阳性,检出率为62.1%。二者联合判读检出率为85.7%。10例尿本周蛋白阳性患者尿液中还检测到了完整免疫球蛋白。97例M蛋白阴性患者中,49例为尿本周蛋白阳性。[结论]血清M蛋白联合尿本周蛋白检测可显著提高多发性骨髓瘤检出率,具有较高的临床价值。[Objective]To investigate the application value of combination detection of serum M-protein and urine Bence Jone proteins(BJP)in multiple myeloma(MM).[Method]IS-CZE and immunofixation electrophoresis were used respectively to detect serum M-protein and urine BJP in 335 patients with MM.The advantages and disadvantages of both methods were analysed.[Result]Serum M-protein were postive in 238 cases(71.0%)among 335 MM patients using IS-CZE,of which 146(61.4%)were IgG type,55(23.1%)were IgA type,5(2.1%)were IgM type and 32(13.4%)were light chain type.Urine BJP were postive in 208 cases(62.1%)among 335 MM patients using urine immunofixation electrophoresis.The sensitivity increased to 85.7%by the combination detection of serum M-protein and urine BJP.Immunoglobulin were also detected in 10 urine BJP-postive patients.Besides,the urine BJP were postive in 49 cases among 97 serum M-protein-negative patients.[Conclusion]The combination detection of serum M-protein and urine BJP can significantly increase the sensitivity of MM and shows great value of clinical application.
