摘　　要：目的:通过比较普通冰冻血浆(FP)、新鲜冰冻血浆(FFP)、普通冰冻血浆和新鲜冰冻血浆混合(FP+FFP)在儿童急性肝功能衰竭血浆置换(PE)中的凝血功能和生化指标,评价FP在急性肝功能衰竭患儿血浆置换中的应用价值。方法:回顾分析59例因急性肝功能衰竭进行PE治疗的患儿临床资料,分为FP置换组(9例),FFP置换组(26例),FP+FFP置换组(24例),比较3组患儿PE治疗前和治疗后的凝血功能和生化指标的差异。结果:3组患儿在PE治疗前凝血功能和生化指标组间比较差异无统计学意义(P>0.05);3组患者在PE治疗后凝血功能和生化指标组间比较差异无统计学意义(P>0.05);患者治疗前后效果组内比较差异有统计学意义(P<0.05),而组间治疗效果差异无统计学意义。结论:在FFP短缺时,FP可以部分或全部代替FFP对急性肝功能衰竭患者进行PE治疗。Objective:To compare the coagulation function and biochemical indicators of normal frozen plasma(FP),fresh frozen plasma(FFP),normal frozen plasma and fresh frozen plasma(FP+FFP)in children with acute liver failure plasma exchange(PE),and evaluate the value of FP in plasma exchange in children with liver failure. Method:The clinical data of 59 patients with PE due to liver failure were retrospectively analyzed.They were divided into FP replacement group(n=9),FFP replacement group(n=26),FP+FFP replacement group(n=24),and three groups of children.The differences in coagulation function and biochemical parameters between before PE treatment and after PE treatment were compared. Result:There were no significant differences between the three groups in the coagulation function and biochemical index before PE treatment(P>0.05).There was no significant difference between the three groups in the coagulation function and biochemical index after PE treatment(P>0.05),the patients in the effect group before and after treatment were statistically significant(P<0.05),so the difference in treatment effect was not statistically significant.Conclusion:In the absence of FFP,FP can partially or completely replace FFP for PE treatment in patients with liver failure.
