摘　　要：目的评价洼田饮水试验筛检头颈部肿瘤患者吞咽障碍风险的效果。方法检索外文数据库:Pubmed、Embase、Ovid、EBSCO host、Web of Science、Science Direct、Cochrane Liberary,中文数据库:维普、CBM、CNKI、万方,检索从建库至2018年11月文献中使用洼田饮水试验筛检头颈部肿瘤患者吞咽障碍风险的诊断、筛检试验。文献质量评价采用"诊断准确性研究的质量评价工具(QUADAS)",资料分析采用meta-disc 1.4。结果共纳入5项筛检试验,筛检头颈部肿瘤患者383例,合并敏感度为66%,合并特异度为65%,受试者诊断特征(SROC)曲线下面积(AUC)为0.7422。结论洼田饮水试验筛检头颈部肿瘤患者吞咽障碍时,其敏感度与特异度一般,但具有一定的准确度,可作为初步筛查工具,但需与其他筛查工具联合使用提高敏感度,避免漏诊。Objective To evaluate the effect of water intake test in screening the risk of dysphagia in patients with head and neck tumors.Methods The foreign language database including Pubmed,Embase,Ovid,EBSCO host,Web of Science,science Direct,Cochrane Liberary,and the Chinese database including vip,CBM,CNKI and wanfang were retrieved.Literatures on the diagnosis and screening test of swallowing disorder risk of patients with head and neck tumor were searched from the construction of the database to Nov.2018.The literature quality was assessed by using the quality assessment of diagnostic accuracy studies(QUADAS)for the study of diagnostic accuracy,and the data were analyzed by using the meta-disc 1.4.Results A total of 5 screening tests were included covering 383 patients with head and neck tumors,with a combined sensitivity of 66%and a combined specificity of 65%.The area under the SROC curve(AUC)of the subjects was 0.7422.Conclusions The sensitivity and specificity of Kubota drinking test in screening dysphagia of patients with head and neck tumors are general,but it has certain accuracy.It can be used as a preliminary screening tool,but it should be combined with other screening tools to improve the sensitivity and to avoid missed diagnosis.
