摘  要:目的比较微阵列蛋白芯片联合定量检测肺癌标志物癌胚抗原(CEA)、细胞角蛋白19片段(CYFRA21-1)、神经特异性烯醇化酶(NSE)及胃泌素释放肽前体(Pro-GRP)与电化学发光法定量测定单指标的相关性,为蛋白芯片检测方法临床应用提供依据。方法应用微阵列蛋白芯片法和电化学发光法同时测定300份肺癌患者血清中CEA、Cyfra21-1、NSE及Pro-GRP含量,比较其检验结果的相关性。定性资料应用配对卡方检验,定量资料应用配对t检验和线性相关分析。结果应用化学发光法微阵列蛋白芯片法与电化学分析法定量检测CEA、Cyfra21-1、NSE及Pro-GRP的线性相关系数R2分别为0.9886,0.9968,0.9972和0.9967,两者结果经配对t检验分别为0.581,0.791,0.424和0.789,P>0.05。以各单项指标大于参考值上限为阳性,低于参考值上限为阴性做配对卡方检验,结果(χ2,P)分别为CEA(0.848,0.581);Cyfra21-1(0.872,0.791);NSE(0.899,0.424);Pro-GRP(0.0714,0.789)。结论微阵列蛋白芯片法与电化学发光法检测肺癌4项肿瘤标志物实验结果和临床诊断均具有良好的相关性,可为肺癌高危人群筛查、疗效评估和预后提供更简便、经济的新检测手段。Objective The aim of this study was to compare the correlation between chemiluminescence microarray protein chip and eletrochemiluminescence immunoassay in detecting lung cancer markers CEA,Cyfra211,NSE,and ProGRP,in order to provide the reference for the clinical application of protein chip detection.Methods The levels of CEA,Cyfra21-1,NSE and Pro-GRP in serum of 300 patients with lung cancer were determined by chemiluminescence microarray protein chip and eletrochemiluminescence immunoassay.Combined with clinical analysis,correlation of the test results was compared.Qualitative data was applied to the paired chi-square test,and the quantitative data was analyzed using paired T test and linear correlation analysis.Results The linear relationship R1 of CEA,Cyfra211,NSE and ProGRP were 0.9886,0.9968,0.9972 and 0.9967,respectively.The results of pairing T test were 0.581,0.791,0.424 and 0.789(P>0.05)respectively by chemiluminescence microarray protein chip and eletrochemiluminescence immunoassay.The matched chi-square test was performed with each individual TM greater than the upper limit of the reference value as positive and negative below the upper limit of the reference value.The results(%2,P)were respectively as follows:CEA 0.84&0.581;Cyfra21-10.872,0.791;NSE 0.899,0.424;ProGRP 0.0714,0.789.Conclusion There was no significant difference and there was a good correlation between chemiluminescence microarray protein chip and electrochemical photoluminescence in the detection of four tumor markers in lung cancer.At the same time,there was no significant difference in clinical diagnosis between the two methods,and the results were consistent.The chemiluminescence microarray protein chip had the advantages of multi-TMs parallel detection,less sample consumption and the like,and provided a new and more convenient and economical detection means for screening,curative effect evaluation and prognosis of the high-risk population of the lung cancer.
