摘　　要：目的了解评价低发区农村上消化道癌早诊早治效果。方法采用整群抽样方法随机选取2015-2019年黑龙江省巴彦县和肇源县目标人群,年龄40~69岁,对符合条件的受检者采用内镜碘染色观察和指示性活检,计算检出率、早诊率等指标,对筛查结果进行分析。结果2015-2019年共筛查8018人,共检测出上消化道癌42例,连续4年的早诊率分别为56%、33%、31%和45%。检出食管原位癌/癌11例,贲门癌4例以及胃原位癌/癌27例。筛查人群随着年龄增加,检出率也呈升高趋势。通过复查随访可以延缓上消化道病变进展。结论在农村上消化道癌低发区通过长期复查随访,可以减缓上消化道癌癌变的进展,对于提高筛查人群的生存时间和生活质量具有重要意义。Objective To evaluate the effectiveness of early diagnosis and treatment of upper digestive tract cancers in rural areas with low incidence of the cancers.Methods Using cluster sampling,we recruited rural residents aged 40–69 years in two counties with lower upper digestive tract cancer incidence than the national level in Heilongjiang province for screening and treatment of the cancer from 2015 to 2019.Gastric endoscopy,iodine staining,and indicative biopsy were adopted in the screening.The effectiveness of the screening was evaluated by detection rate and early diagnosis rate.Results During 2015–2019 among a total of 8018 participants,42 upper gastrointestinal cancer cases were detected(including 11 cases of carcinoma/in situ carcinoma of esophagus,4 cases of cardia carcinoma,and 27 cases of carcinoma/in situ carcinoma of stomach)and the rate of early diagnosis for the four consecutive screenings was 56%,33%,31%and 45%,respectively.The detection rate increased with the increment of age among the participants.The cancer-related upper digestive tract lesions were retarded among the participants under follow-up treatment.Conclusion Screening and follow-up treatment are effective in control and treatment of upper digestive tract cancers among residents in rural regions with low incidence of the cancers.
