帝视内窥镜引导双腔支气管导管插管在纵隔肿瘤切除手术中的应用

摘　　要：目的观察帝视内窥镜引导双腔支气管导管插管在纵隔肿瘤切除手术中的临床应用效果。方法选取2017年1月~2018年12月我院收治的30例纵隔肿瘤切除手术患者作为研究对象,采用随机数字表法将其分为帝视内窥镜组(D组)和GlideScope视频喉镜组(G组),每组各15例。D组患者采用帝视内窥镜引导插入气管,G组患者采用视频喉镜辅助插管。比较两组患者的插管时间、一次插管成功率、定位时间及麻醉诱导前(T1)、麻醉诱导后(T2)、插管成功即刻(T3)、插管后5 min(T4)血流动力学指标[平均动脉压(MAP)、心率(HR)]。结果两组患者T1、T2的MAP、HR比较,差异无统计学意义(P>0.05);两组患者T2、T3、T4的MAP、HR均低于本组T1,差异有统计学意义(P<0.05);D组患者T3、T4的MAP、HR均低于G组,差异有统计学意义(P<0.05)。两组患者的一次插管成功率比较,差异无统计学意义(P>0.05);D组患者的插管时间、定位时间均短于G组,差异有统计学意义(P<0.05)。结论帝视内窥镜引导双腔支气管导管插管用时短,且插管时对患者心血管系统影响较小。Objective To observe the application clinical effect of double-lumen tracheal tube intubation guided by Discopo in the mediastinal tumor resection.Methods Thirty patients with mediastinal tumor resection in our hospital from January 2017 to December 2018 were selected as the research subjects,and they were divided into the Discopo group(group D)and the GlideScope video laryngoscope group(group G)according to the random number table method,15 cases in each group.The patients in group D were guided by Discopo for intubation,and the patients in group G were assisted by video laryngoscope.The intubation time,the success rate of one intubation,the positioning time,and the hemodynamic indicators(mean arterial pressure[MAP],heart rate[HR])before anesthesia induction(T1),after anesthesia induction(T2),immediate successful intubation(T3),and 5 minutes after intubation(T4)were compared between the two groups.Results There were no significant differences in MAP and HR at T1 and T2 between the two groups(P>0.05).The MAP and HR in the two groups at T2,T3,and T4 were lower than those at T1,and the differences were statistically significant(P<0.05).The MAP and HR at T3 and T4 in group D were lower than those in group G,and the differences were statistically significant(P<0.05).There was no significant difference in the success rate of one intubation between the two groups of patients(P>0.05).The intubation time and positioning time were shorter in group D than those in group G,and the differences were statistically significant(P<0.05).Conclusion The intubation time of double-lumen tracheal tube intubation guided by Discopo is short,and the impact on the cardiovascular system of patients is small when intubating.
