腰椎间盘突出症臭氧髓核消融术治疗中远期疗效观察

摘　　要：目的腰椎间盘突出症所导致的腰腿痛严重影响患者生活质量,而传统手术治疗并发症多,近年来臭氧髓核消融术治疗椎间盘突出症的疗效明显。本研究探讨臭氧髓核消融术治疗腰椎间盘突出症的中远期(≥10年)效果。方法选取2005-12-26-2007-01-20南阳市骨科医院行臭氧消融手术治疗26例腰椎间盘突出症术后3个月恢复良好的患者为研究对象。通过电话随访和门诊调查的形式分析患者中远期疗效。结果手术治疗10年后,整体疗效为显效,其中治愈1例(4.0%),显效15例(58.0%),有效3例(11.0%),无效7例(27.0%),总有效率为73.1%。治疗10年后,腰痛评分总分为(23.58±5.03)分,高于治疗前的(15.31±5.10)分,差异有统计学意义,t=12.442,P<0.001。其中,腰背痛评分为(2.38±0.75)分,高于治疗前的(0.96±0.87)分,t=11.280,P<0.001;腿痛或麻评分为(2.15±0.88)分,高于治疗前的(0.88±0.76)分,t=9.706,P<0.001;步行能力评分为(2.27±1.04)分,高于治疗前的(1.04±0.77)分,t=8.208,P<0.001;直腿抬高评分为(1.92±0.39)分,高于治疗前的(1.31±0.68)分,t=4.924,P<0.001;运动障碍评分为(1.81±0.57)分,高于治疗前的(1.31±0.62)分,t=5.000,P<0.001;ADL受限评分为(11.04±2.41)分,高于治疗前的(7.85±2.29)分,t=11.743,P<0.001。结论初步研究结果提示,臭氧髓核消融术治疗椎间盘突出症的中远期疗效较好。OBJECTIVE Lumbar and leg pain caused by lumbar disc herniation seriously affects the patients,quality of life,and there are many complications in traditional surgical treatment.In recent years,medical ozone nucleus pulposus ablation has a significant effect on the treatment of lumbar disc herniation.This study was to investigate the medium and long term(10 years)effect of medical ozone nucleus pulposus ablation.METHODS Total 26 patients with lumbar intervertebral disc herniation who had been treated by ozone ablation in Nanyang Orthopaedic Hospital from December 26,2005 to January 20,2007 were selected and their long-term efficacy was analyzed by telephone follow-up and outpatient investigation.RESULTS After 10 years of operation,the overall curative effect was remarkable,including 1(4.0%),15(58.0%),3(11.0%)were effective,7(27.0%)were ineffective,and the total effective rate was 73.1%.After 10 years of treatment,the total score of low back pain(23.58±5.03)was higher than that before treatment(15.31±5.10),t=12.442,P<0.001,the difference was statistically significant.In addition,the low back pain score after treatment(2.38±0.75)was higher than(0.96±0.87,t=11.280,P<0.001)before treatment.The leg pain or anesthesia score after treatment(2.38±0.75)was higher than(0.96±0.87,t=11.280,P<0.001)before treatment.The walking ability score after treatment(2.38±0.75)was higher than(0.96±0.87,t=11.280,P<0.001)before treatment.The straight leg elevation score after treatment(1.92±0.39)was higher than(1.31±0.68,t=4.924,P<0.001)before treatment.The dyskinesia score after treatment(1.81±0.57)was higher than(1.31±0.62,t=5.000,P<0.001)before treatment.The ADL restricted score after treatment(11.04±2.41)was higher than(7.85±2.29,t=11.743,P<0.001)before treatment.CONCLUSION The medium and long term effect of ozone nucleus pulposus ablation in the treatment of intervertebral disc herniation is better.
