肾上腺节细胞神经瘤的超声特征及误诊分析

摘　　要：目的探讨肾上腺节细胞神经瘤的超声特征。方法分析经术前超声诊断或术后病理证实的肾上腺节细胞神经瘤12例患者临床资料,观察病灶的回声特点及彩色血流信号。结果12例病灶均为单发,右侧肾上腺9例,左侧肾上腺3例,大小于2.1~12.3cm。3例呈类圆形或椭圆形,4例呈不规则分叶状,5例呈“嵌入形”生长,3例向下生长包绕下腔静脉。12例病灶均为低回声,6例内见强回声,手术后证实为钙化灶,4例内见少量点片状彩色血流信号。12例中8例术前超声诊断为节细胞神经瘤与病理相符,2例误诊为肾上腺囊肿,1例误诊为腺瘤,1误诊为嗜铬细胞瘤。结论超声对肾上腺节细胞神经瘤的诊断具有重要价值,但少部分节细胞神经瘤缺乏特征表现或与其它病变超声表现近似,容易误诊,需要鉴别诊断。Objective To investigate the ultrasound features of adrenal ganglioneuroma and the analysis of misdiagnosed cases so as to improve the preoperative diagnosis of the disease.Methods 12 cases of adrenal ganglioneuroma diagnosed by preoperative ultrasound or confirmed by pathology from January 2014 to June 2019 were retrospectively analyzed.The echo characteristics,growth pattern,shape and size of the lesion and color flow signals were observed,and the sonographic features were summarized.The causes of misdiagnosis were analyzed.Results In 12 cases,9 cases were found in the right adrenal gland and 3 cases in the left.All the tumors were solitary and the diameter was between 2.1~12.3 cm.3 cases were round or oval,4 cases were irregular lobulated,5 cases were"embedded"growth,3 cases were downward growth around the inferior vena cava.12 lesions were hypoechoic,6 lesions had strong internal echo,calcification was confirmed after operation,and 4 lesions showed a small amount of dot-like color flow signal.In those 12 cases,8 cases were diagnosed as ganglioneuroma by preoperative ultrasonography,2 cases were misdiagnosed as adrenal cyst,1 case was misdiagnosed as adenoma and 1 case was misdiagnosed as pheochromocytoma.Conclusion Ultrasound is of great value in the diagnosis of adrenal ganglioneuroma,but a small number of ganglion neuroma lacks characteristic or similar to other lesions,which is easy to be misdiagnosed and requires differential diagnosis.
