小儿胰腺肿瘤18例临床分析

摘　　要：目的探讨小儿胰腺肿瘤的诊断与手术方法。方法回顾性分析2002年10月—2018年10月我科收治的胰腺肿瘤病儿18例的临床资料。18例病儿均行相关影像学检查,均发现胰腺囊实性包块。根据肿瘤部位选择相应手术方式。结果保留幽门胰十二指肠切除10例,肿瘤局部切除1例,肿瘤切除、胆总管空肠ROUX-Y吻合1例,胰体尾+脾切除6例。18例病儿术后随访至今,除1例促纤维增生性小圆细胞瘤病儿术后死亡外,其余未见异常。结论小儿胰腺肿瘤临床上较罕见,不同病理类型临床表现不同,手术为其主要治疗方法。Objective To discuss the diagnosis and operations for pancreatic tumor in children.Methods The clinical data of 18 children with pancreatic tumor who were admitted to our department from October 2002 to October 2018 were retrospectively analyzed.Cystic-solid masses were found in all patients’pancreas by imaging examination.Surgical procedures were chosen according to the tumor location.Results Ten patients underwent pylorus-preserving pancreatoduodenectomy.One patients underwent local resection.One patient underwent tumor resection and ROUX-Y choledochojejunostomy.Six patients underwent distal pancreatectomy with splenectomy.Among the 18 cases followed up,one with desmoplastic small round cell tumor died after operation,and the rest showed no abnormalities.Conclusion Pancreatic tumors in children are relatively rare in clinical practice,and their clinical manifestations vary between different pathological types.Surgery is the main treatment for pancreatic tumors in children.
