影响精神分裂症患者韦氏记忆量表评分的相关性分析

摘　　要：目的探究影响精神分裂症患者韦氏记忆量表(WMS-Ⅳ)评分的相关因素。方法选取2018年1月~12月青海省第三人民医院收治的精神分裂症患者118例,并选取同期健康者118例作为健康对照组。所有研究对象均采用WMS-Ⅳ评估记忆功能,采用磁共振成像(MRI)检查大脑结构,采用基于体素的形态测量法(VBM)分析大脑的相应解剖结构的差异,采用基于体素的空间分析处理(TBSS)提取研究对象大脑结构的弥散各向异性分数(FA)值,探究影响患者WMS-Ⅳ评分的显著相关因素。结果精神分裂症患者的WMS-Ⅳ各项分量表评分以及总记忆商均显著低于健康人群,差异有统计学意义(P <0.05)。受教育时间、病程、用药时间、非典型抗精神病药物使用量、阳性和阴性症状量表(PANSS)评分结果均不同程度的与患者的WMS-Ⅳ评分总记忆商以及各分量表评分存在显著的相关性。健康人群中,左侧海马旁回以及海马体是影响听觉记忆指数的显著相关因素,左侧海马旁回、左侧梭状回以及海马体是影响总记忆商的显著相关因素,但在精神分裂症患者中,脑结构与WMS-Ⅳ评分结果间不存在显著相关性。结论受教育时间、病程、用药时间、非典型抗精神病药物使用量、PANSS评分结果均是影响精神分裂症患者WMS-Ⅳ评分相关性因素。Objective To investigate the factors that correlated with the WMS-Ⅳ score in patients with schizophrenia.Methods 118 patients diagnosed with schizophrenia in our hospital from January 2010 to December 2018 were enrolled in the study.118 healthy controls were selected as the control.All subjects underwent WMS-Ⅳ to assess the memory function,MRI was used to examine brain structure,voxel-based morphometry(VBM)and(Tract-based spatial statistics,TBSS)were used to investigate the relationship between the brain structure and WMS-Ⅳ score. Results The scores of WMS-Ⅳ in patients with schizophrenia were significantly lower than those of healthy controls.The level of education,duration of illness,duration of medication use,atypical antipsychotic use,and PANSS scores were significantly correlated with the WMS-Ⅳ score.In healthy controls,the left parahippocampal gyrus and hippocampus were correlated with the score of auditory memory index.The left parahippocampal gyrus,left fusiform and hippocampus were significantly correlated with the total memory index,but there was no correlations in patients with schizophrenia.Conclusion The level of education,duration of disease,time of administration,the dose of atypical antipsychotic use,and PANSS scores are factors which related with the WMS-Ⅳ score in patients with schizophrenia.
