11C-CFT PET/CT显像与帕金森病综合评定量表评分相关性分析

摘　　要：目的探讨11C-CFT PET/CT显像与帕金森病综合评定量表(UPDRS)评分和运动功能部分(UPDRSⅢ)评分的相关性,并进一步探索将多模态影像参数纳入帕金森病临床评分体系中的科学性及可行性。方法回顾性选取2015年2月~2018年4月于解放军总医院核医学科进行11C-CFT PET/CT确诊的23例帕金森病患者,同时选取23例健康受试者为健康对照组,分析两组临床数据及PET/CT图像,通过勾画感兴趣体积计算尾状核和壳核的DAT结合量指数,并与临床UPDRS评分和UPDRSⅢ评分进行相关性分析。结果尾状核DAT结合量指数与UPDRS总分、UPDRSⅢ评分间均呈负相关(r=-0.526,-0.472),且与UPDRS总评分的相关性更强;壳核DAT受体结合量指数与UPDRS总分、UPDRSⅢ评分间呈负相关(r=-0.483,-0.565),且与UPDRSⅢ评分的相关性更强。结论11C-CFT PET/CT显像可客观评估帕金森患者病情的严重程度,建议考虑将多模态影像参数纳入帕金森病的临床评分体系中。Objective To investigate the correlation between PET/CT DAT imaging and Parkinson’s disease UPDRS(comprehensive rating scale)score and UPDRS Ⅲ (motor function rating scale)score.By doing so,we seek to figure out if we could put the multimodality image parameters into clinical scoring practice.Methods The clinical data and PET/CT images of 23 patients with Parkinson’s disease and 23 well-controlled healthy subjects were retrospectively analyzed.Region of Volume are delineated to calculate the DAT binding index in the caudate nucleus and putamen,and then respectively analyze the correlation between DAT binding index and clinical scoring system-UPDRS and UPDRS Ⅲ--in the striatum.Results In the caudate nucleus and putamen,DAT binding indexes were all negatively associated with the UPRADS total score and UPDRS Part Ⅲ score,whose differences were both found statistically significant(P < 0.05);Moreover,a closer correlation between the index and UPDRS total score can be seen in the caudate nucleus(r1=-0.526,P1=0.01;r2=-0.472,P2=0.23).Yet,in the putamen,the indexes were more associated with UPDRS PartⅢscores(r1=-0.483,P1=0.02;r2=-0.565,P2=0.005).Conclusion PET/CT DAT imaging can serve as an objective indicator of the patients’ advancement with Parkinson’s disease.Furthermore,the multimodality image parameters could probably be considered as part of into clinical scoring system.
