人工晶状体植入联合超声乳化吸出术治疗急性闭角型青光眼的观察

摘　　要：目的观察人工晶状体植入联合超声乳化吸出术治疗急性闭角型青光眼的临床效果。方法选取114例急性闭角型青光眼患者为研究对象,随机数字表法分为观察组和对照组,各57例(各57眼)。对照组行小梁切除术,观察组行人工晶状体植入联合超声乳化吸出术治疗。观察2组手术前后眼压、视力、前房深度及并发症发生情况。结果术前2组眼压、视力及前房深度比较,差异无统计学意义(P>0.05);术后观察组眼压(12.21±3.57)mmHg,低于对照组(14.52±5.14)mmHg;视力(0.48±0.21)、前房深度(3.16±1.05)mm,高于对照组的(0.32±0.18)、(2.36±0.97)mm,差异有统计学意义(P<0.05)。观察组并发症总发生率5.26%,低于对照组的19.30%,差异有统计学意义(P<0.05)。结论急性闭角型青光眼患者接受人工晶状体植入术联合超声乳化吸出术治疗并发症少,且降眼内压效果较理想,利于改善患者视力与前房深度。Objective To observe the clinical efficacy of phacoemulsification combined with intraocular lens implantation in the treatment of acute angle-closure glaucoma.Methods 114 patients with acute angle-closure glaucoma treated were selected as the research objects and were divided into the observation group and the control group by the random number table method,with 57 cases in each group.The control group was treated with trabeculectomy,while the observation group with phacoemulsification combined with intraocular lens implantation.The intraocular pressure,vision,anterior chamber depth before and after the surgery and the complications in the two groups were observed.Results Before the surgery,there was no statistically significant difference in the intraocular pressure,vision and anterior chamber depth between the two groups before the surgery(P>0.05).After the surgery,the intraocular pressure in the observation group[(12.21±3.57)mmHg]was lower than that in the control group[(14.52±5.14)mmHg],while the vision and anterior chamber depth in observation group[(0.48±0.21),(3.16±1.05)mm]were higher than those in the control group[(0.32±0.18),(2.36±0.97)mm](P<0.05).The total incidence of complications in the observation group(5.26%)was lower than that in the control group(19.30%),and the difference was statistically significant(P<0.05).Conclusion There are fewer complications of the phacoemulsification combined with intraocular lens implantation in the treatment of acute angle-closure glaucoma,and better effects of lowering the intraocular pressure,which is conducive to improving the vision and anterior chamber depth of patients.
