摘　　要：目的探讨血清降钙素原(PCT)水平与急性脑梗塞(ACI)患者脑水肿严重程度的相关性。方法选取ACI患者160例,根据NIHSS评分标准分为轻度脑梗塞患者62例、中度脑梗塞患者54例和重度脑梗塞患者44例,另选取健康体检者102例为对照组,测定ACI患者入院第2 d、3 d、5 d、7 d空腹血清PCT水平及患侧大脑半球脑电阻抗扰动系数(CEI)。结果ACI组患者在入院第2 d患侧CEI高于对照组,且随时间呈增高趋势,患侧CEI于第5 d达到峰值,第7 d开始出现下降趋势,差异均有统计学意义(P<0.05);ACI患者血清PCT水平与患侧CEI变化趋势基本一致;ACI患者各时间点血清PCT水平与患侧CEI扰动系数值均呈正相关(P<0.05);重度ACI组血清PCT水平和患侧CEI高于轻度和中度ACI组,且中度ACI组高于轻度ACI组,差异均有统计学意义(P<0.05),各组血清PCT水平和患侧CEI均明显高于对照组(P<0.05)。结论ACI患者血清PCT水平升高,与ACI患者脑水肿严重程度密切相关。Objective To explore the relationship between serum procalcitonin(PCT)level and severity degree of cerebral edema in patients with acute cerebral infarction(ACI).Methods 160 patients with ACI were divided into three groups according to NIHSS scores:the mild ACI group(n=62),the moderate ACI group(n=54)and the severe ACI group(n=44).102 healthy people were enrolled in our study as the control group.Serum PCT level and the turbulence coefficient of electric impedance of cerebral hemisphere(CEI)were determined in ACI at 2 d,3 h,5 h and 7 h after hospitalization.Results Patients with ACI had higher CEI value after admission 2 days compared with the control group;CEI value increased in the following days,reaching a peak of about the first five days,but it decreased at the seventh day,with the differences statistically significant(P<0.05).ACI patients had a substantially consistent trend in the serum PCT level and CEI value;serum PCT level was positively correlated with CEI value in each time point in patients with ACI(P<0.05).Serum PCT level and CEI value were higher in the severe ACI group compared with the mild and moderate ACI groups,which were higher in the mild than moderate ACI group,with the differences statistically significant(P<0.05).All ACI groups had higher serum PCT level and CEI value than the control group(P<0.05).Conclusion Serum PCT level is higher in patients with ACI and it significantly correlated with severity degree of cerebral edema.
