人附睾蛋白的联合检测在肺癌诊断中的临床价值

摘　　要：目的探讨血清人附睾蛋白(HE4)与其他肿瘤标志物联合检测在肺癌诊断中的临床价值。方法选取本院初诊的肺癌患者52例作为恶性组、肺部良性疾病患者67例作为良性组、健康体检者60例作为对照组,同时检测血清HE4、CEA、CYFRA21-1、NSE水平。结果恶性组血清HE4、CEA、CYFRA21-1、NSE水平均高于良性组及对照组(P<0.05)。恶性组中四项联合检测阳性率明显高于单项检测。恶性组中HE4的敏感度和AUC均高于其他项目,四者联合检测的敏感度、阴性预测值及AUC均高于单项检测(P<0.05)。结论肺癌患者血清HE4、CEA、CYFRA21-1、NSE水平升高,联合检测有助于肺癌的诊断和对高危人群的筛查,HE4在肺癌诊断方面可能是更好的肿瘤标志物。Objective To investigate the clinical value of combined detection of serum human epididymal protein(HE4)and other tumor markers in the diagnosis of lung cancer.Methods Fifty-two patients with newly diagnosed lung cancer were selected as malignant group,67 patients with benign lung disease as benign group and 60 healthy subjects as control group.The levels of serum HE4,CEA,CYFRA21-1 and NSE were measured.Results The levels of serum HE4,CEA,CYFRA21-1 and NSE in the malignant group were higher than those in the benign group and the control group(P<0.05).The positive rate of four combined tests in malignant group was significantly higher than that in single test.The sensitivity and AUC of HE4 in the malignant group were higher than those in other subjects.The sensitivity,negative predictive value and AUC of the combined detection were higher than the single test(P<0.05).Conclusion The levels of serum HE4,CEA,CYFRA21-1,and NSE in patients with lung cancer are elevated.Combined detection is helpful for the diagnosis of lung cancer and screening for high-risk groups.HE4 may be a better tumor marker for lung cancer diagnosis.
