摘　　要：目的探讨miR-141-3p和miR-22-3p在胰腺癌组织中的表达及其与临床病理特征及预后的关系。方法选择2014年1月-2017年1月在河北省邯郸市中心医院普外三科进行手术治疗的胰腺癌患者92例,取其病灶标本及癌旁组织标本,检测其miR-141-3p、miR-22-3p表达量的差异,进一步根据胰腺癌组织中的表达量中位数分为高表达、低表达组各46例。比较不同miR-141-3p、miR-22-3p表达的胰腺癌患者临床病理特征差异,分析不同miR-141-3p、miR-22-3p表达对患者预后的影响,采用COX多因素风险回归模型分析影响胰腺癌患者预后的危险因素。结果胰腺癌组织中miR-141-3p、miR-22-3p的表达量均低于癌旁组织(t=25.883、42.527,P均<0.01)。不同miR-141-3p表达的胰腺癌患者中TNM分期、肿瘤分化程度、远处转移的分布差异有统计学意义(χ^2=4.246、6.500、4.423,P均<0.05),不同miR-22-3p表达的胰腺癌患者中远处转移的分布差异有统计学意义(χ^2=14.331,P<0.01);miR-141-3p、miR-22-3p低表达组患者的生存率低于高表达组患者(χ^2=6.873、5.999,P均<0.05)。TNM分期较高、肿瘤分化程度较低、远处转移、miR-141-3p低表达、miR-22-3p低表达均是导致胰腺癌患者预后不良的危险因素(RR=2.771、1.781、3.185、1.609、1.717,P均<0.05)。结论胰腺癌组织中miR-141-3p、miR-22-3p均呈低表达,可能参与疾病发生发展,且在疾病预后的评估方面具有一定意义。Objective To investigate the expression of miR-141-3 p and miR-22-3 p in pancreatic cancer and their relationship with clinicopathological features and prognosis.Methods From January 2014 to January 2017,ninety two patients with pancreatic cancer were selected for surgical treatment in general surgery Department of Handan Central Hospital in Hebei Province.The focus samples and the adjacent tissue samples were taken.The expression of miR-141-3 p and miR-22-3 p were detected.According to the expression of pancreatic cancer tissue,46 cases were divided into high expression group and low expression group.The clinicopathological characteristics of pancreatic cancer patients with different expression of miR-141-3 p and miR-22-3 p were compared.To analyze the influence of different expression of miR-141-3 p and miR-22-3 p on the prognosis of patients.Cox multivariate risk regression model was used to analyze the risk factors that affect the prognosis of patients with pancreatic cancer.Results The expression levels of miR-141-3 p and miR-22-3 p in pancreatic cancer were lower than those in the adjacent tissues(t=25.883,t=42.527,P<0.01).There were significant differences in TNM stage,tumor differentiation and distant metastasis among the patients with different miR-141-3 p expression(χ^2=4.246,χ^2=6.500,χ^2=4.423,P<0.05),The distribution of distant metastasis in patients with different expression of miR-22-3 p was statistically significant(χ^2=14.331,P<0.01).The survival rate of patients with low expression of miR-141-3 p and miR-22-3 p was lower than that of patients with high expression(χ^2=6.873,χ^2=5.999,P<0.05).High TNM stage,low tumor differentiation,distant metastasis,low expression of miR 1413 p,and low expression of miR-22-3 p were all risk factors for poor prognosis of pancreatic cancer patients(RR=2.771,RR=1.781,RR=3.185,RR=1.609,RR=1.717,P<0.05).Conclusion The expression of miR-141-3 p and miR-22-3 p in pancreatic cancer is low,which may be involved in the occurrence and development of the disease.
