芪参还五胶囊结合立体定向引流术对出血性中风患者神经功能的影响

摘　　要：目的:探究芪参还五胶囊结合立体定向引流术对出血性中风患者神经功能影响。方法:选取2016年5月至2018年5月沧州中西医结合医院收治的出血性中风患者82例作为研究对象,按照就诊顺序编号随机分为对照组和观察组,每组41例。对照组给予常规西药治疗,观察组给予芪参还五胶囊结合立体定向引流术治疗,比较2组治疗前、后Bartiel量表评分、改良Rankin量表评分、NIHSS评分、PRO评分、血液流变学指标、炎性因子水平;治疗后比较2组总有效率,治疗期间药物不良反应发生率。结果:1)治疗后2组Bartiel量表评分较治疗前均显著升高,改良Rankin量表评分、NIHSS评分、PRO评分指标均显著下降(P<0.05),且观察组均较对照组改善效果更优(P<0.05);2)治疗后2组患者全血高切黏度、全血低切黏度、血浆黏度、血细胞比容、纤维蛋白原均较治疗前显著下降(P<0.05),且观察组低于对照组(P<0.05);3)治疗后2组患者IL-4、TNF-a、CRP较治疗前均显著下降,且观察组显著低于对照组(P<0.05);4)观察组总有效率高于对照组(P<0.05);5)2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:芪参还五胶囊结合立体定向引流术有助于恢复出血性中风神经功能,促进血液循环,抑制炎性反应,能提高疗效。Objective:To explore effects of Qishen Huanwu Capsules combined with stereotactic drainage on neurological function in patients with hemorrhagic stroke.Methods:A total of 82 patients with hemorrhagic stroke admitted in Cangzhou Hospital of Integrated Traditional Chinese Medicine and Western Medicine from May 2016 to May 2018 were randomly divided into control group(41 cases)and observation group(41 cases)according to the number of visit.The control group was treated with conventional western medicine,and the observation group was treated with Qishen Huanwu Capsules combined with stereotactic drainage.Bartiel scale scores,modified Rankin scale scores,NIHSS scores,PRO scores,hemorheology indexes and inflammatory factor levels before and after the treatment were compared between the 2 groups;effective rates and adverse drug reaction rates in the 2 groups were compared after the treatment.Results:1)After the treatment,the scores of Bartiel scale in the 2 groups were significantly higher than those before the treatment,and the scores of modified Rankin scale,NIHSS and PRO were significantly decreased(P<0.05),and the improvement in the observation group was better than that in the control group(P<0.05).2)After the treatment,the whole blood high shear viscosity,whole blood low shear viscosity,plasma viscosity,hematocrit and fibrinogen in the 2 groups were significantly lower than those before the treatment(P<0.05),and those in the observation group were lower than those in the control group(P<0.05).3)After the treatment,IL-4,TNF-αand CRP in the 2 groups were significantly lower than those before the treatment,and those in the observation group were significantly lower than those in the control group(P<0.05).4)The total effective rate in the observation group was higher than that in the control group(P<0.05).5)There was no significant difference in adverse reactions between the 2 groups(P>0.05).Conclusion:Qishen Huanwu Capsules combined with stereotactic drainage is helpful to restore the neurological function in hemorr
