经皮球囊扩张椎体后凸成形术治疗多发性骨髓瘤导致椎体骨折的临床效果

摘　　要：目的:观察多发性骨髓瘤导致椎体病理性骨折的患者应用经皮球囊扩张椎体后凸成形术的临床疗效,分析及总结安全性。方法:回顾2013年1月-2019年1月笔者所在医院运用PKP(经皮椎体后凸成形术)方法治疗的37例(共计40个椎体)多发性骨髓瘤导致压缩性骨折患者临床效果,随访观察患者术前、术后1 d、术后1个月、术后6个月的VAS评分、Barthel指数记分及椎体高度。结果:PKP手术成功率100%;术后31例患者自觉疼痛症状明显改善或消失;5例患者自觉症状部分改善;1例患者自觉症状较术前无明显改善,分别占患者总数的83.8%、13.5%、2.7%,总有效率达97.3%。术后VAS评分、Barthel指数记分明显优于术前(P<0.05)。术后椎体前缘、中份高度明显较术前恢复(P<0.05)。1例患者出现骨水泥外渗,仅有影像学改变,无严重并发症发生。结论:运用经皮球囊扩张椎体后凸成形术治疗多发性骨髓瘤导致椎体病理性骨折的患者,具有症状缓解明显的特点,明显提高了患者的生活质量。Objective:To observe the clinical efficacy of percutaneous balloon kyphoplasty for patients with pathological fracture of vertebral body caused by multiple myeloma,and analyze and summarize the safety.Method:From January 2013 to January 2019,37 patients(40 vertebrae in total)with compression fracture caused by multiple myeloma treated by PKP(percutaneous vertebral kyphoplasty)were reviewed.The VAS score,Barthel index before operation,1 day after operation,1 month and 6 months after operation and vertebral height before operation,and 6 months after operation were evaluated.Result:The success rate of PKP was 100%.The symptoms of conscious pain were improved or disappeared in 31 patients after operation.The conscious symptoms were partly improved in 5 cases.There was no obvious improvement in 1 case,which accounted for 83.8%,13.5%,2.7% of the total,and the total effective rate was 97.3%.VAS score and Barthel index score after operation were significantly better than before(P<0.05).The anterior and middle height of the vertebral body recovered significantly after operation(P<0.05).Bone cement exosmosis was found in one patient with only imaging changes and no serious complications.Conclusion:Percutaneous balloon kyphoplasty for the treatment of multiple myeloma patients with pathological fractures of the vertebral body has characteristic of the symptom relief obvious and significantly improved the quality of life of patients.
