摘 要:目的探讨蝶骨电极联合长程视频脑电图对颞叶癫痫灶定位诊断的价值。方法对临床疑诊的140例颞叶癫痫患者依次行常规脑电图(REEG)、视频脑电图(VEEG)、蝶骨电极+VEEG检查的结果进行比较分析,其定位结果与术中深部脑电图(DEEG)结果进行比较。结果痫样放电在REEG、VEEG、蝶骨电极+VEEG的总检出率分别为11.4%,72.1%和90.0%;VEEG、蝶骨电极+VEEG的定侧率为66.3%和88.9%(P<0.01)。术中DEEG证实了术前的蝶骨电极+VEEG定位准确率高,DEEG与蝶骨电极+VEEG定位吻合率达96.4%.结论蝶骨电极+VEEG对颞叶癫痫致痫灶的检出率和定侧率高,且致痫灶切除准确率高,可提高手术切除治疗颞叶癫痫的疗效。Objective To explore the value of sphenoidal electrodes combined with the long-term video-electroencephalography(VEEG) in locating temporal lobe epilepsy.Methods The results of the examination were compared from the routine electroencephalogram(REEG),VEEG and sphenoidal electrodes+VEEG were performed in 140 patients who were suspected temporal lobe epilepsy.The positioning results of the above examinations were compared with the results of deep-electroencephalogram(DEEG) during operation.Results The total detection rates of epileptiform discharges in REEG,VEEG and sphenoid electrodes+VEEG were 11.4%,72.1% and 90.0% respectively.The lateral fixation rate of VEEG and sphenoidal electrode+VEEG was 66.3% and 88.9%(P<0.01).The deep-electroencephalogram DEEG during operation confirmed that it had high accuracy of positioning with sphenoidal electrodes+VEEG before surgery.The anastomosis rate of DEEG with the sphenoidal electrodes+VEEG reached 96.4%.Conclusion The sphenoidal electrodes+VEEG has higher detection rate and higher lateral rate in epileptogenic foci of temporal lobe epilepsy,and a high accuracy rate of epilepsy resection,which can improve the curative effect of surgical resection for temporal lobe epilepsy.
