摘 要:目的评价应用18F-FDG PET-CT、癌胚抗原(CEA)、糖抗原199(CA199)联合检测对结直肠癌(CRC)诊断的有效性。方法对2012年1月~2019年9月收治的119例CRC患者的临床资料进行回顾性分析,所有患者均接受18F-FDG PET-CT检查、血清CEA和CA199检测。在PET-CT检查中,根据病变区域计算最大标准摄取值(SUV)。SUV值≥2.5,血清CEA>3.4μg/L,CA199>27U/ml可诊断为恶性肿瘤。分析18F-FDG PET-CT,CEA,CA199联合检测CRC的诊断效率及价值。结果 CRC患者SUV,CEA,CA199的检出率明显高于正常人(P<0.01),且SUV,CEA和CA199值均高表达。三模式联合检测CRC的准确性为92.23%,敏感性为96.87%,特异性为87.00%,优于单一模式(P<0.05)。结论联合应用18F-FDG PET-CT,CEA,CA199诊断CRC具有较高的准确性、敏感性和特异性,是高危人群及特殊群体的CRC检测及评估的好方法。Objective To evaluate the diagnostic value of the combination of 18F-FDG PET-CT,carcinoembryonic antigen(CEA),and carbohydrate antigen(CA) 199 in detection of colorectal cancer(CRC).Methods From January 2012 to November 2019,119 patients confirmed as CRC by biopsy were recruited for this retrospective analysis.They received 18F-FDG PET-CT examination,CEA,and CA199 tests.Additionally,40 volunteers were also enrolled.During the PET-CT examination,maximum standard uptake value(SUV) of lesions was calculated according to the region of interest of lesion site and SUV≥2.5 was adjudicated as the presence of malignant tumor.Serum CEA>3.4μg/L and CA199>27 U/ml were adjudicated as positive CRC results.The diagnostic efficiency of combining 18F-FDG PET-CT,CEA,and CA199 in detecting CRC were analyzed.Results The results of SUV,CEA,and CA199 in CRC patients were remarkably higher than those in normal volunteers(all P<0.01).SUV,CEA,and CA199 were highly expressed in patients.Accuracy of the three-modality combination in detection of CRC was 92.23%,sensitivity was 96.87%,and specificity was 87.00%,which were superior to those of the single modality(P<0.05).Conclusion The combined application of 18F-FDG PET-CT,CEA,CA199 for diagnosis of CRC has high accuracy,sensitivity and specificity,and it is worthy to be widely used in the examination and evaluation of colorectal cancer in high-risk and special groups.
