多层螺旋CT评价特发性腹膜后纤维化炎症活动性可行性的初步研究

摘　　要：目的探讨多层螺旋CT(MSCT)评价特发性腹膜后纤维化(IRPF)炎症活动性的可行性。方法收集26例IRPF患者的临床及MSCT资料。回顾性分析治疗前后患者红细胞沉降率(ESR)、C反应蛋白(CRP)和MSCT影像变化及相关性。结果获得复查的10例患者治疗后ESR、CRP、Δ动、Δ静、最大平均厚度、最大左右径均低于治疗前,差异均有统计学意义(P<0.05或0.01);患者治疗前后平扫MSCT值、动脉期MSCT值、静脉期MSCT值比较差异均无统计学意义(均P>0.05)。Δ动、Δ静与ESR、CRP均呈正相关(均P<0.05)。平扫MSCT值、动脉期MSCT值、静脉期MSCT值、最大平均厚度、最大左右径与ESR、CRP均无相关性(均P>0.05)。ΔA、ΔB与ΔESR、ΔCRP均呈正相关(r=0.38、0.54、0.49、0.41,均P<0.05)。结论MSCT增强检查可以评价IRPF炎症活动性,并且可以评价IRPF的疗效。IRPF病变的大小与炎症活动性无相关性。Objective To investigate the feasibility of multi-slice spiral CT(MSCT)in diagnosis of inflammatory activity of idiopathic retroperitoneal fibrosis(IRPF).Methods The clinical data of 26 patients with IRPF diagnosed in the Affiliated Hospital of Hangzhou Normal University and the Second Affiliated Hospital of Dalian Medical University from January 2013 to June 2019 were retrospectively analyzed.The MSCT scans were performed and the ESR and CRP were measured before and after treatment;the correlation of MSCT with CRP and ESR levels was analyzed.Results After treatment,ESR,CRP,ΔArterial,ΔVenous,the maximum average thickness,the maximum left-right diameter of 10 patients were significantly lower than those before treatment(P<0.05 or 0.01);while there was no significant difference in MSCT value of plain scan,arterial phase and venous phase before and after treatment(P>0.05).TheΔArterial andΔVenous were positively correlated with ESR and CRP levels(P<0.05).There was no correlation between the MSCT values of plain scan,arterial phase,venous phase,maximum average thickness,maximum left-right diameter and ESR,CRP(all P>0.05).TheΔA andΔB were correlated withΔESR andΔCRP(r=0.38,0.54,0.49,0.41,respectively;all P<0.05).Conclusion The inflammation activity of IRPF can be evaluated by enhancement range of MSCT value,and the curative effect of IRPF can be evaluated intuitively.There is no correlation between the size of IRPF lesions on MSCT and the inflammation activity.
