摘 要:目的通过检测肺结核患者血清中miR-16和miR-155的表达量,探讨其作为肺结核辅助诊断和鉴别诊断指标的临床应用价值。方法应用实时荧光定量PCR法(qRT-PCR)检测72例肺结核患者、50例肺癌患者和45名健康人血清中miR-16和miR-155的表达量,分析其在肺结核检测中的灵敏度、特异度、阳性预测值和阴性预测值。结果肺结核患者血清中miR-16的表达量显著高于健康人组和肺癌组,差异具有统计学意义(F=5.564,P<0.01);而肺结核患者血清中miR-155的表达量显著低于健康人组和肺癌组,差异具有统计学意义(F=2.563,P<0.05);血清miR-16和miR-155在肺结核中检测的灵敏度分别为44.4%和41.7%、特异度分别为88.4%和84.2%、阳性预测值分别为74.4%和66.7%、阴性预测值分别为67.7%和65.6%。二者联合对肺结核检测的灵敏度明显提高,达到了56.9%,与单项miR-16、miR-155比较,差异具有统计学意义(x^2=4.003,P<0.05)o结论肺结核患者血清中miR-16和miR-155的检测对于肺结核的辅助诊断及鉴别诊断具有一定的临床参考价值。Objective To evaluate the clinical significance for diagnosis of detection of serum miR-16 and miR-155 in pulmonary tuberculosis patients(PTB).Methods Real-time quantitative PCR method was used to examine the serum miR-16 and miR-155 levels in 72 PTB patients,50 lung caner patients and 45 healthy controls.The sensitivity,specificity,positive predictive value and negative predictive value of diagnosis for PTB were evaluated.Results The level of miR-16 in patients with PTB was significantly higher than that of healthy controls and lung caner patients(F=5.564,P<0.01).The level of miR-155 was significantly lower than that of healthy controls and lung caner patients(F=2.563,P<0.05).The sensitivity of miR-16 and miR-155 in PTB were 44.4% and 41.7% respectively,the specificity were 8&4%and 84.2%,the positive predictive value were 74.4%and 66.7%,and the negative predictive value were 67.7% and 65.6%.The specificity was 56.9%after the combination of miR-16 and miR-155 in PTB,which was significantly higher than that of single miRNA(x^2=4.003,P<0.05).Conclusion The detection of miR-16 and miR-155 in serum has clinical value in aid diagnosing PTB.
