养正消积胶囊联合铂类化疗方案治疗非小细胞肺癌的Meta分析

摘　　要：目的系统评价养正消积胶囊联合铂类药物为基础的化疗方案治疗非小细胞肺癌(NSCLC)的有效性,为临床实践提供证据。方法计算机检索Cochrane图书馆,中国期刊全文数据库(CNKI),维普中文科技期刊全文数据库(VIP),万方数据库(WanFang Data)和中国临床试验注册中心,检索时限从建库至2019年5月,纳入有关养正消积胶囊联合铂类化疗方案治疗晚期NSCLC的随机对照试验,根据Cochrance Handbook5.1评价标准和工具评价纳入文献质量,提取资料并核对后,采用RevMan5.3软件进行Meta分析。结果纳入8个随机对照试验(RCT),共951例患者,Meta分析结果显示与以铂类药物为基础的单纯化疗组相比,联用养正消积胶囊能显著提高临床治疗有效率[RR=1.22,95%CI:1.08~1.38,P=0.001],改善患者生活质量[RR=1.63,95%CI:1.34~1.97,P<0.00001]和中医证候评分[RR=2.85,95%CI:1.58~5.12,P=0.0005],降低化疗引起的白细胞、血小板、血红蛋白下降等骨髓抑制的发生。结论以铂类药物化疗为主的NSCLC患者应用养正消积胶囊可提高治疗有效率,改善生活质量及中医证候评分,降低骨髓抑制的发生。Objective To systematically evaluate the effectiveness of Yangzheng Xiaoji capsule combined with platinum-based chemotherapy regimen in the treatment of non-small cell lung cancer(NSCLC),and to provide evidence for clinical practice.Methods The Cochrane Library,VIP,Wan-fang Database,CNKI and China Clinical Trials Registry were searched by computer.The retrieval time was from the establishment of the database to May 2019.Randomized controlled trials of Yangzheng Xiaoji Capsule combined with platinum chemotherapy for advanced non-small cell lung cancer were included.According to Cochrance Handbook 5.1 evaluation criteria and tools,the quality of the literature was evaluated,and the data were extracted and checked.After that,RevMan 5.3 software was used for Meta-analysis.Results:8 randomized controlled trials involving 951 patients were included.Meta-analysis showed that the combination of Yangzheng Xiaoji Capsule and platinum-based chemotherapy group could significantly improve the efficiency of clinical treatment[RR=1.22,95%CI(1.08,1.38),P=0.001],improve the quality of life of patients[RR=1.63,95%CI(1.34,1.97),P<0.00001]and TCM syndrome score[RR=2.85;95%CI(1.58,5.12),P=0.0005],and reduce leukocyte,blood caused by chemotherapy.The occurrence of bone marrow suppression such as the decrease of white blood cells,platelets and hemoglobin caused by chemotherapy was reduced.Conclusion The application of Yangzheng Xiaoji Capsule in patients with non-small cell lung cancer treated mainly with platinum drugs can improve the treatment efficiency,improve the quality of life and TCM syndrome score and reduce the incidence of bone marrow depression.
