摘 要:目的分析咬肌腮腺区海绵状血管瘤的超声影像特征,与病理组织学对比,探讨其超声成像的病理基础,提高彩色多普勒超声诊断腮腺区海绵状血管瘤的准确率。方法应用彩色多普勒超声高频探头对咬肌腮腺区包块进行探测,分析46例经手术后病理证实的咬肌腮腺区海绵状血管瘤彩色多普勒超声下显示包膜、大小、形态、边界、位置、后方回声、内部回声特点、与腮腺的关系,彩色多普勒血流信号及病理组织学特点。结果肿块均为位于咬肌区或腮腺内,超声声像图大致分为3型:①蜂窝状多囊性肿块;②高回声光带分隔成较细小网格状肿块;③实质性低回声肿块。结论根据咬肌腮腺区海绵状血管瘤特征性超声声像图,发现其病理构成不同组成成分,分类进行彩色多普勒超声鉴别。Objective to analyze ultrasonographic characteristics of cavernous hemangioma in parotid region of masseter muscle,and explore pathological basis of ultrasonographic imaging,to improve accuracy of color Doppler ultrasound in diagnosis of cavernous hemangioma in parotid region.Methods detect parotid gland mass with high frequency probe of color Doppler ultrasound.Analyze cavernous hemangioma in parotid gland area of of 46 cases masseter confirmed by pathology after operation.Color Doppler ultrasound showed characteristics of capsule,size,shape,boundary,position,back echo,internal echo,relationship with parotid gland,color Doppler blood flow signal and histopathological characteristics.Results masses were located in masseter area or parotid gland,and sonograms were divided into three types:1 honeycomb like mass;2 high echo light band separated into smaller grid like mass;3 solid low echo mass.Conclusion according to characteristic ultrasound image of cavernous hemangioma in parotid gland area of masseter muscle,it is found different components of its pathological structure,and classify them with color Doppler ultrasound.
