血清TNF-α、IL-35在炎症性肠病患者中的表达及临床价值

摘　　要：目的探讨血清肿瘤坏死因子-α(TNF-α)、白介素-35(IL-35)在炎症性肠病(IBD)患者中的表达及临床价值。方法选取2016年9月至2018年9月收治的IBD患者48例,其中克罗恩病(CD)患者24例为CD组、溃疡性结肠炎(UC)患者24例为UC组,同期选取体检健康人员48例为对照组,所有研究对象均检测血清TNF-α、IL-35水平。结果CD组、UC组血清TNF-α水平明显高于对照组,CD组、UC组血清IL-35水平明显低于对照组,差异有统计学意义(P<0.05),CD组、UC组血清TNF-α、IL-35水平比较,差异无统计学意义(P>0.05);CD组、UC组中,活动期患者血清TNF-α水平明显高于缓解期患者,活动期患者血清IL-35水平明显低于缓解期患者,差异有统计学意义(P<0.05),CD组和UC组活动期患者、缓解期患者血清TNF-α、IL-35水平比较,差异无统计学意义(P>0.05);Pearson相关性分析结果显示,血清TNF-α与IL-35呈负相关(r=-0.842,P<0.05)。结论血清TNF-α、IL-35水平与CD、UC等IBD的发生发展有关,提示TNF-α、IL-35可作为评估IBD及其病情进展的重要指标。Objective To investigate the expression and clinical significance of serum tumor necrosis factor-alpha(TNF-αlpha)and interleukin-35(IL-35)in patients with inflammatory bowel disease(IBD).Methods A total of 48 patients with IBD who were admitted and treated in our hospital from September 2016 to September 2019 were enrolled in the study,who were divided into Crohn's disease group(CD group,n=24)and ulcerative colitis group(UC group,n=24),at the same time,the other 48 healthy subjects were enrolled as control group.The serum levels of TNF-αlpha and IL-35 were detected and compared among the three groups.Results The serum levels of TNF-a CD group and UC group were significantly higher than those in control group,and the serum levels of IL-35 in CD group and UC group were significantly lower than those in control group(P<0.05),however,there were no significant differences in the serum levels of TNF-αand IL-35 between CD group and UC group(P>0.05).In CD group and UC group,the serum levels of TNF-αat active stage were significantly higher than those at remission stage,the serum levels of IL-35 levels in active stage were significantly lower than those in remission stage(P<0.05).However there were no significant differences in the serum levels of TNF-αand IL-35 at active stage and remission stage between CD group and UC group(P>0.05).In addition Pearson correlation analysis results showed that the serum levels of TNF-αlpha were negatively correlated with those of IL-35(P<0.05).Conclusion The serum levels of TNF-αand IL-35 are related to the pathogenesis and development of IBD,which suggests that TNF-a and IL-35 can be regarded as important indexes in evaluating IBD and disease condition development.
