摘  要:目的研究幽门螺旋杆菌L型(Helicobacter pylori L-form,Hp-L型)感染与巨噬细胞移动抑制因子(Macrophage migration inhibitor,MIF)、基质金属蛋白酶9(Matrix metalloproteinase 9,MMP9)和血管内皮生长因子(Vascular endothelial growth factor,VEGF)表达水平变化关系。方法选取2016年9月-2018年7月南阳医学高等专科学校第一附属医院慢性胃炎患者84例为胃炎组、胃溃疡患者60例为溃疡组、胃癌患者52例为胃癌组,同期再选取健康体检者125名为对照组。观察和记录四组研究对象Hp-L型感染阳性率、Hp-L型感染程度和MIF、MMP9、VEGF表达水平。结果胃癌组Hp-L、MIF、MMP9、VEGF阳性表达率分别为73.08%、67.31%、94.23%、90.38%,高于溃疡组的56.67%、51.67%、80.00%、78.33%,溃疡组高于胃炎组的47.62%、42.86%、51.19%、52.38%,胃炎组高于对照组的7.20%、5.60%、26.40%、37.60%,差异均有统计学意义(P<0.05)。胃炎组、溃疡组、胃癌组Hp-L型感染重度者MIF、MMP9、VEGF阳性表达率分别高于同组中度者,中度者高于轻度者,差异均有统计学意义(P<0.05)。Hp-L型感染严重程度分别与MIF、MMP9、VEGF阳性表达率呈正相关关系,差异有统计学意义(P<0.05)。结论 Hp-L型感染能够增加MIF、MMP9、VEGF阳性表达,加重胃黏膜损伤程度,促进了胃癌病变发生;降低Hp-L型感染率,监测MIF、MMP9、VEGF阳性表达状况,利于早期诊断、全面把握病情,避免癌变发生。OBJECTIVE To study the relationship between Helicobacter pylori L-form(Hp-L type) infection and the expression of macrophage migration inhibitor(MIF), matrix metalloproteinase 9, MMP9 and vascular endothelial growth factor(Vascular endothelial growth factor, VEGF). METHODS Subjects in the first affiliated hospital of Nanyang Medical College from September 2016 to July 2018 were recruited in this study. Eighty-four patients with chronic gastritis were divided into the gastritis group;60 patients with ulcer were in the ulcer group;52 patients with gastric cancer were in the gastric cancer group;125 healthy volunteers were enrolled in the control group. The positive rate of Hp-L infection, the intensity of Hp-L infection and the expression levels of MIF, MMP9 and VEGF among the groups were observed and compared. RESULTS The positive expression rates of Hp-L, MIF, MMP9 and VEGF in the gastric cancer group(73.08%、67.31%、94.23%、90.38%) were significantly higher than that in the ulcer group(56.67%、51.67%、80.00%、78.33%), which was significantly higher than that in the gastritis group(47.62%、42.86%、51.19%、52.38%), which was also significantly higher than that in the control group(7.20%、5.60%、26.40%、37.60%)(P<0.05). The positive rates of MIF, MMP9 and VEGF in the gastritis group, ulcer group and gastric cancer group with severe Hp-L infection were significantly higher than that in the moderate group, which were higher than that in the mild group(P<0.05). The severity of Hp-L infection was positively correlated with the positive expression rates of MIF, MMP9 and VEGF, respectively(r=0.895,0.713,0.405,P<0.05). CONCLUSION Hp-L infection can increase the positive expression rates of MIF, MMP9 and VEGF, and aggravate the degree of gastric mucosal damage and promote the occurrence of gastric cancer. It must be an effective way of reducing the infection rate of Hp-L and detecting the expression of MIF, MMP9 and VEGF to conduct the early diagnosis and comprehensively recognize the disease, so as to avoid th
