摘 要:目的探索腹腔镜与开腹根治手术下胃癌患者的氧化应激、血清炎症因子及感染状况。方法选取2016年7月-2018年7月于中国石化集团胜利石油管理局胜利医院接受治疗的124例患者为研究对象,按照随机数表法将患者平均分为两组,62例实施腹腔镜下胃癌根治术进行治疗的患者为试验组,62例采用开腹胃癌根治术进行治疗的患者为对照组。检测并比较两组患者的丙二醛(Malondialdehyde,MDA)、晚期氧化蛋白产物(Human advanced oxidation protein products,AOPP)、谷胱甘肽过氧化物酶(Glutathione peroxidase,GSH-Px)、超氧化物岐化酶(Superoxide Dismutase, SOD)氧化应激反应指标和白细胞介素-2(Interleukin-2,IL-2)、白细胞介素2可溶性受体(Soluble interleukin-2 receptor,SIL-2R)血清炎性因子水平,比较两组患者的并发症总发生率。结果术后试验组患者的MDA(26.25±3.51)u/L、AOPP(53.71±6.38)μmol/L水平低于对照组患者(P<0.05);试验组患者GSH-Px(214.62±37.26)μg/ml、SOD(15.46±4.29)μg/ml水平高于对照组患者(P<0.05);术后试验组患者的IL-2(32.48±4.12)μg/ml和SIL-2R(23.66±4.58)μg/ml水平低于对照组患者(P<0.05);试验组患者的并发症总发生率为8.06%,低于对照组患者的25.81%(P<0.05)。结论腹腔镜胃癌根治术在保证疗效的同时能够有效抑制炎性因子的活化和氧化应激反应的发生,降低了感染率和并发症发生率,改善了患者的预后质量,且具有微创、术后恢复快等优点。OBJECTIVE To explore the oxidative stress, serum inflammatory factors and infection status in patients with gastric cancer under laparoscopic and open radical surgery. METHODS A total of 124 patients who were treated in the Shengli Hospital of Shengli Petroleum Administration Bureau of Sinopec Group from July 2016 to July 2018 were enrolled. The patients were divided into two groups according to the random number table method. patients underwent laparoscopic gastric cancer. Sixty-two patients who underwent radical surgery were in the experimental group, and 62 patients who underwent radical gastrectomy were in the control group. The levels of malondialdehyde(MDA), advanced oxidation protein products(AOPP), glutathione peroxidase(GSH-Px), superoxide Dismutase(SOD) oxidative stress response indicators and interleukin-2(IL-2), interleukin-2 soluble receptor(SUL-2 R) serum inflammatory Factor levels in both groups were detected and compared to reveal the correlation between the overall incidence of complications and these indexes. RESULTS The content of MDA(26.25±3.51) u/L and AOPP(53.71±6.38) μmol/L in the postoperative group were significantly lower than that in the control group(P<0.05). The levels of GSH-Px(214.62±37.26) μg/ml and SOD(15.46±4.29) μg/ml in the experimental group were significantly higher than that in the control group(P<0.05). The levels of IL-2(32.48±4.12) μg/ml and SIL-2 R(23.66±4.58) μg/ml in the postoperative group were significantly lower than that in the control group(P<0.05). The overall incidence of complications in the observation group was 8.06%, which was significantly lower than that in the control group with the percentage of 25.81%(P<0.05). CONCLUSION Laparoscopic radical gastrectomy can effectively inhibit the activation of inflammatory factors and oxidative stress to reduce the infection and complication rate, and to improve the prognosis quality of patients.
