摘 要:目的探讨Sysmex XE-5000自动化血液分析仪的中性粒细胞参数(NEUT-X中性粒细胞的侧向散射光、NEUT-Y中性粒细胞荧光)在鉴别急性髓细胞白血病和急性淋巴细胞白血病中的应用价值。方法收集急性白血病初发患者87例(急性白血病组)的外周血液样本,其中62例为急性髓细胞白血病组,25例为急性淋巴细胞白血病组,另收集80例健康体检者(对照组)的外周血样本,用Sysmex XE-5000上对其NEUT-X、NEUT-Y参数进行检测。结果急性白血病组与对照组相比,NEUT-X、NEUT-Y 2个参数结果均有降低,差异有统计学意义(P <0. 05)。急性髓细胞白血病组NEUT-X、NEUT-Y值均较对照组降低,差异有统计学意义(P <0. 05);急性髓细胞白血病组NEUT-X值与急性淋巴细胞白血病组NEUT-X值比较降低,差异有统计学意义(P <0. 05),NEUT-Y值差异无统计学意义。结论中性粒细胞参数NEUT-X、NEUT-Y值对急性白血病的鉴别有价值;NEUT-X值对鉴别急性髓细胞白血病与急性淋巴细胞白血病的价值更大。Objective To investigate the clinical value of NEUT-X,NEUT-Y parameters in differentiating acute myelogenous leukemia and acute lymphoblastic leukemia. Methods The neutrophil NEUT parameters( NEUT-X,NEUT-Y) of peripheral blood samples from 62 cases of acute myelogenous leukemia,25 cases of acute lymphocytic leukemia and 80 healthy individuals were analyzed on Sysmex XE-5000 hematology analyzer. Results There are significant decreases in the neutrophil NEUT parameters( NEUT-X,NEUT-Y) from the acute leukemia group compared with control group,and the differences were statistically significant( P < 0. 05). The NEUT-X and NEUT-Y values in the acute myeloid leukemia group decreased compared with the control group,with the differences statistically significant( P < 0. 05). NEUT-X decreased in the acute myeloid leukemia group compared with the acute lymphocytic leukemia group,and the difference was statistically significant( P < 0. 05),but there was no statistical significance on the differences of NEUT-Y. Conclusion NEUT-X and NEUT-Y parameters of neutrophils are valuable for the differentiation between acute leukemia and healthy individuals. The value of NEUT-X may be more valuable in differentiating acute myeloid leukemia from acute lymphoblastic leukemia.
