计算机认知矫正治疗与日间康复技术对精神分裂症患者认知功能的改善作用比较

摘　　要：目的比较计算机认知矫正治疗(CCRT)与日间康复技术对精神分裂症患者认知功能的改善作用。方法将60名精神分裂症患者随机分为康复组和认知组,各30例。在常规药物治疗的基础上,康复组患者采用日间康复治疗,认知组患者接受CCRT,疗程均为12周。比较两组患者治疗前后的阳性与阴性症状量表(PANSS)评分、计算机化成套认知功能测评(ACCT)评分。结果治疗12周后,康复组PANSS阴性症状评分及总评分均低于治疗前及认知组(均P<0.05)。认知组ACCT的连线时间、数字正序的评分均优于治疗前及康复组(均P<0.05)。结论CCRT与日间康复技术均能够在一定程度改善精神分裂症患者的认知功能,但两种方法改善的侧重点略有不同。建议针对患者的具体情况,将两种方法相结合,或选择最适合患者的康复模式。Objective To compare the effects of computerized cognitive remediation therapy(CCRT)versus daytime rehabilitation technology on cognitive function improvement in schizophrenics.Methods Sixty schizophrenics were randomly divided into rehabilitation group and cognitive group,with 30 cases in each group.On the basis of conventional medication,patients in the rehabilitation group were treated by daytime rehabilitation,while patients in the cognitive group received CCRT,with a treatment course of 12 weeks in both groups.Pre-and post-treatment Positive and Negative Symptom Scale(PANSS)scores and A Battery of Computerized Cognitive Test(ACCT)scores were compared between the two groups.Results Twelve weeks after treatment,the rehabilitation group had lower scores for PANSS negative symptoms as well as lower total score than pre-treatment and the cognitive group(all P<0.05).Compared to pre-treatment and the rehabilitation group,the cognitive group exhibited superior scores for the items of ACCT,including on-line time and positive digital sequence(all P<0.05).Conclusion Both CCRT and daytime rehabilitation technology can improve the cognitive function of schizophrenics to a certain extent,but the two methods have their own focuses which are slightly different from each other.It is suggested to combine the two methods or choose the optimum rehabilitation mode for patients in accordance with patients’specific conditions.
