恩替卡韦与替诺福韦对非霍奇金淋巴瘤合并乙型肝炎病毒感染患者肝功能保护效果的比较

摘　　要：目的比较恩替卡韦与替诺福韦对非霍奇金淋巴瘤(NHL)合并HBV感染患者肝功能的保护效果。方法将72例NHL合并HBV感染患者根据随机数字表法分为对照组与观察组,每组36例。两组患者均给予常规化学治疗,在化疗前1周,对照组予以恩替卡韦治疗,观察组予以替诺福韦治疗。比较两组患者化疗期间HBV-DNA及ALT水平,观察两组患者化疗进程及肝炎相关病死率。结果治疗后6周、12周、18周,观察组的ALT及HBV-DNA水平均低于对照组(均P<0.05),两组患者ALT及HBV-DNA水平均有随时间变化的趋势(均P<0.05);两组化疗中断、延期化疗和继续按期化疗情况差异均无统计意义(均P>0.05),但观察组肝炎相关病死率低于对照组(P<0.05)。结论与恩替卡韦比较,NHL合并HBV感染患者化疗期间采用替诺福韦进行抗病毒治疗的效果更好。Objective To compare the protective effects of entecavir versus tenofovir disoproxil on liver function in patients with non-Hodgkin lymphoma(NHL)complicated with HBV infection.Methods Seventy-two patients with NHL complicated with HBV were divided into control group and observation group according to the random number table method,with 36 cases in each group.Patients in both groups were given routine chemotherapy,before one week of treatment,the control group received entecavir and the observation group received tenofovir disoproxil.The levels of HBV-DNA and ALT during chemotherapy were compared between two groups,and chemotherapy outcome and hepatitis-related mortality were observed in both groups.Results Six,12 and 18 weeks after chemotherapy,the levels of ALT and HBV-DNA in the observation group were lower than those in the control group(all P<0.05),ALT or HBV-DNA level exhibited a tendency to change with time in both groups(all P<0.05);no statistically significant difference was observed between the two groups in chemotherapy discontinuance,delayed chemotherapy or regular chemotherapy continuance(all P>0.05),but hepatitis-related mortality in the observation group was lower than that in the control group(P<0.05).Conclusion Compared with entecavir,tenofovir disoproxil applied to antiviral therapy achieves better efficacy for patients with NHL complicated with HBV infection during chemotherapy.
