糖尿病合并肺结核患者前白蛋白、视黄醇结合蛋白水平及其与营养状况的相关性研究

摘　　要：目的探讨糖尿病合并肺结核患者前白蛋白、视黄醇结合蛋白(RBP)水平及其与营养状况的相关性。方法选取100例糖尿病合并肺结核患者,根据营养风险筛查2002将其分为有营养风险组、无营养风险组,每组50例,再选取50名健康体检者作为正常体检组。比较3组体质指数、血红蛋白、白蛋白、前白蛋白、RBP和超氧化物歧化酶(SOD)水平,分析糖尿病合并肺结核患者前白蛋白、RBP与体质指数、血红蛋白、白蛋白、SOD的相关性。结果有营养风险组的体质指数、血红蛋白、白蛋白、前白蛋白、RBP和SOD水平均低于无营养风险组与正常体检组(均P<0.05);无营养风险组的白蛋白、前白蛋白、RBP水平均低于正常体检组(均P<0.05);糖尿病合并肺结核患者血清前白蛋白与体质指数、血红蛋白、RBP、SOD均呈正相关(均P<0.05),血清RBP与体质指数、血红蛋白、白蛋白、SOD均呈正相关(均P<0.05)。结论糖尿病合并肺结核患者血清前白蛋白、RBP水平均降低,且均与营养状况指标呈正相关,检测其水平可以发现早期营养不良患者。Objective To investigate the prealbumin and retinol-binding protein(RBP)levels and their correlation with nutritional status in diabetes mellitus patients complicated with pulmonary tuberculosis.Methods A total of 100 diabetes mellitus patients complicated with pulmonary tuberculosis enrolled were divided into nutritional risk group and non nutritional risk group according to Nutritional Risk Screening 2002,with 50 cases in each group,and 50 healthy check-up individuals were enrolled as normal check-up group.The level of body mass index(BMI),hemoglobin,ALB,prealbumin,RBP and superoxide dismutase(SOD)were compared among the three groups,and the correlation of prealbumin or RBP with BMI,hemoglobin,ALB or SOD was analyzed in diabetes mellitus patients complicated with pulmonary tuberculosis.Results The levels of BMI,hemoglobin,ALB,prealbumin,RBP and SOD in the nutritional risk group were lower than those in the non nutritional risk group and normal check-up group(all P<0.05).The levels of ALB,prealbumin and RBP in the non nutritional risk group were lower than that in the normal check-up group(all P<0.05).Among diabetes mellitus patients complicated with pulmonary tuberculosis,serum prealbumin positively correlated with BMI,hemoglobin,RBP or SOD(all P<0.05),and serum RBP positively correlated with BMI,hemoglobin,ALB or SOD(all P<0.05).Conclusion Decreased levels of serum prealbumin and RBP are observed in diabetes mellitus patients complicated with pulmonary tuberculosis,and positively correlate with nutritional status indicators.Detection of these two indicators levels can identify patients with early malnutrition.
