全脱位恒牙再植后的临床疗效

摘　　要：目的:观察全脱位恒牙再植后的临床疗效。方法:纳入7~15岁因外伤致全脱位患儿34例42颗恒牙,牙根发育完成者行根管治疗(n=25),牙根发育未完成者保留牙髓(n=17),进行再植。术后每3~6个月进行临床和影像学检查。再植后复查期6~66个月。结果:治疗成功23颗牙,再植牙牙周膜愈合;治疗失败19颗牙,其中13颗牙齿固连或替代性吸收,6颗牙牙根炎性吸收。牙齿离体时间少于60 min者成功率高于离体时间大于60 min者(72.22%vs 41.67%,P<0.05);经根管治疗者成功率大于未经根管治疗者(68.00%vs 35.29%,P<0.05)。结论:对全脱位恒牙缩短离体时间和牙根发育完成并经根管治疗后再植有利于提高再植后牙周膜愈合率。Objective:To evaluate the clinical effects of replantation of avulsed permanent teeth.Methods:42 avulsed permanent teeth in 34 patients(7-15 years old)were treated with replantation.The teeth with mature roots were treated by root canal therapy(RCT,n=25)and thoes with immature roots were not(n=17),then replaced.The clinical examinations and X-ray examination were performed after replantation.The patients were followed up for 6-66 months.Results:Healing with a normal periodontal ligament was observed in 23 teeth,healing with ankylosis or substitutive root resorption in 13 teeth,and inflammatory resorption in 6 teeth.The success treatment ratio of the teeth pretreated by RCT was higher than that of without RCT(68.00%vs 35.29%,P<0.05).The success rate of tooth replantation after 15-60 min in vitro was higher than that after>60 min(72.22%vs 41.67%,P<0.05).Conclusion:Decrease of the in vitro durrition and RCT for the teeth with mature roots may increase the sccess ratio of replacement of the avalsed permanent teeth.
