摘  要:目的:建立隐形矫治与固定矫治有限元模型并模拟临床推上颌磨牙远移,分析其位移趋势以及应力分布的差异。方法:通过CT扫描获得自愿者(男,25岁)上颌骨数据,利用Mimics等软件建立隐形矫治器与固定矫治器的三维模型,施加模拟隐形及固定矫治器推上颌第二磨牙远移的载荷,比较其位移趋势及其牙周膜应力分布差异。结果:隐形组磨牙远移时位移的旋转中心位于根尖1/3处,固定组位移的旋转中心位于根分叉处;牙周膜最大应力值隐形组高于固定组,但隐形组应力分布均匀;支抗前牙位移隐形组小于固定组。结论:隐形矫治与固定矫治在推磨牙远移时均使磨牙产生远中倾斜移动趋势,在对于支抗牙的控制方面隐形组优于固定组。Objective:To analyze the displacement trend and stress distribution of the maxillary molar distalization under 2 different stress modes by a three-dimensional finite element model.Methods:The maxillary data of a volunteer(male,25 years old)were obtained through CT scan and were reconstructed with Mimics,a three-dimensional model of invisible appliance and fixed appliance was established with corresponding softwares.The displacement trend of maxillary second molars and the Von Mises stress distribution of pericementum were compared under 2 different loading modes by respectively applying simulated loads of invisible appliance and fixed appliance for maxillary second molar distalization.Results:When simulating the load of invisible appliance,the rotation centers of the second molars were located at 1/3 of the apex,that was at the root furcation when fixed appliance was used.The maximum stress of invisible appliance group was higher than that of fixed appliance group,but the stress distribution in the invisible group was more uniform than that of fixed appliance group.The displacement of anchorage anterior teeth in the invisible appliance group was smaller than that in the fixed appliance group.Conclusion:Both invisible and fixed appliance may make the distal inclination of molars by pushing the molars distally.The invisible appliance is more effective for the control of anchorage teeth than the fixed appliance.
