46例颈动脉体瘤诊治单中心经验

摘　　要：目的:总结颈动脉体瘤的临床表现和治疗经验。方法:回顾性分析46例颈动脉体瘤的临床资料、手术方式及随访结果。46例患者Shamblin分类,其中Ⅰ型2例,Ⅱ型37例,Ⅲ型7例。年龄15~74岁,平均47.8岁,男∶女=1∶5.57。左侧14例,右侧30例,2例为双侧。颈淋巴结转移1例。家族史1例,三代共计5人发病,其中1人为双侧。结果:单纯动脉外膜下剥离38例。动脉外膜下剥离合并颈外动脉切除6例。颈总动脉分歧部切除2例:其中1例未吻合血管,1例切除后行颈总-颈内动脉吻合。术后均无脑缺血症状、体征。颅神经损伤8例。ShamblinII型术后24 h内脑梗塞2例,无手术相关死亡。随访复发2例。结论:尽管颈动脉体瘤手术治疗总体是安全的,仍有发生颅神经损伤、术后脑卒中的可能性。Objective:To summarize the clinical manifestation and treatment experience of carotid body tumor(CBT).Methods:46 cases of carotid body tumors were treated.There were 7 men and 39 women(male∶female ratio=1∶5.57),with a mean age of 47.8 years(15-74 years).Left sided CBT was in 14 cases,right in 30 cases and bilateral in 2 cases.Cervical lymph node metastasis was in 1 case.A case was with family history,5 patients of 3 generations were found,including one bilateral case.Results:38 cases were treated by subadventitia dissection,6 by external-carotid artery excision combined with subadeventitia dissection.The bifucation of common carotid was resected in 2 cases,1 of them without subsequent vascular reconstruction,another 1 was treated by the anastomosis of the common carotid artery with the internal carotid artery.No symptoms and signs of cerebral ischemia was observed after operation in these 2 cases.There were 8 cases of cranial nerve injury and 2 cases of cerebral infarctions within 24 h postoperatively identified.2 cases of recurrence were found during postoperative follow-up.Conclusion:Commonly,the surgical treatment of carotid body tumor is effective and safe.Although the risk of cranial nerve injury and postoperative stroke remains.
