精神分裂症患者高危阶段与首发阶段事件相关电位(ERP)的对照研究

摘　　要：目的:探索精神分裂症患者高危阶段和首发阶段事件相关电位(ERP)的特征。方法:应用德国Brain Products公司生产的ERP仪器,采集并比较健康正常组(健康组)、精神分裂症高危阶段组(高危组)和首发阶段组(首发组)患者的ERP检测结果各20例。结果:①健康组、高危组和首发组ERP外源性成分N1、P2潜伏期延长(F=5.04,4.05;P<0.05),P2波幅逐渐降低(F=8.81,P<0.01),内源性成分N2潜伏期延长(F=5.45,P<0.05),差异具有统计学意义;②健康组、高危组和首发组MMN潜伏期、S1P50潜伏期逐渐延长,S2P50波幅降低,差异有统计学意义(F=39.69,9.07,5.13;P<0.01)。结论:精神分裂症患者高危阶段和首发阶段均存在认知功能缺陷。事件相关电位ERPs部分成份(N1、N2、P2、MMN及S1P50的潜伏期延长、P2及S2P50的波幅降低)可作为精神分裂症高危阶段和首发阶段认知功能缺陷动态监测和随访的重要生物学评价指标。Objective:To explore the characteristics of event-related potentials(ERP)in high-risk and initial stages of schizophrenia.Methods:By using ERP instruments of Brain Products Company(Germany),the ERP test was conducted on 20 cases in healthy normal group(healthy group),high-risk group(high-risk group)and first-stage group(first-stage group)of schizophrenia,respectively.Results:①The latency of exogenous components N1 and P2 of ERP in healthy group,high-risk group and first-stage group was prolonged(F=5.04,4.05,P<0.05),the amplitude of P2 gradually decreased(F=8.81,P<0.01),and extended latency of endogenous component N2(F=5.45,P=0.01).②In the healthy group,high-risk group and first-stage group,the latency of MMN and S1 P50 were gradually prolonged(F=39.69,9.07,P<0.01),and the S2 P50 amplitude was decreased(F=5.13,P<0.01).Conclusion:There are cognitive deficits in schizophrenics at high-risk and initial stages.Some components of event-related potential ERPs(prolonged latency of N1,N2,P2,MMN and S1 P50,decreased amplitude of P2 and S2 P50)can be used as important biological indicators for dynamic monitoring and follow-up of cognitive impairment in high-risk and initial stages of schizophrenia.
