摘 要:目的:探讨磁共振弥散张量成像对成人基底节脑出血患者内囊传导束损伤的临床诊断价值。方法:回顾性分析2016年2月至2018年3月在我院治疗的82例成人基底节脑出血患者的临床资料,所有患者于入院后,均接受CT检查,且需在发病48h内接受磁共振弥散张量成像,同时进一步分析内囊区各向异性(FA)变化情况以及皮质脊髓束(corticospinal system,CST)损伤程度与患肢肌力关系。结果:入院时、治疗1个月后,患侧FA值均低于健侧,差异有统计学意义(P<0.05);82例患者中,1级12例,2级30例,3级40例。入院时与治疗1个月后,随着CST损伤分级的增加,患者患肢肌力呈递减趋势,差异有统计学意义(P<0.05)。结论:磁共振弥散张量成像可对成人基底节高血压脑出血患者CST受损程度给予准确评估,且可有效预测其患肢肌力情况。Objective: To investigate the clinical value of diffusion tensor imaging( DTI) in the diagnosis of internal capsular conduction tract injury in adult patients with basal ganglia intracerebral hemorrhage. Methods: The clinical data of 82 cases of adult basal ganglia intracerebral hemorrhage treated in our hospital from February 2016 to March 2018 were retrospectively analyzed. All patients received CT examination after admission,and received diffusion tensor magnetic resonance imaging within 48 hours of onset. At the same time,the changes of internal capsular anisotropy( FA) and cortex were further analyzed. The degree of injury of corticospinal system( CST) is related to the muscle strength of the affected limb. Results: At admission and one month after treatment,the FA value of affected side was lower than that of healthy side,with significant difference( P<0.05). Among 82 patients,there were12 cases of grade 1,30 cases of grade 2 and 40 cases of grade 3. At admission and 1 month after treatment,the muscle strength of affected limbs showed a decreasing trend with the increase of CST injury grade( P<0.05). Conclusion: MR diffusion tensor imaging can accurately assess the degree of CST damage in adult patients with basal ganglia hypertensive intracerebral hemorrhage,and can effectively predict the muscle strength of affected limbs.
