莫沙必利联合氟哌噻吨美利曲辛治疗功能性消化不良对患者近端胃舒张及胃排空功能影响分析

摘　　要：目的分析莫沙必利联合氟哌噻吨美利曲辛治疗功能性消化不良(functional dyspepsia,FD)对患者近端胃舒张及胃排空功能的影响。方法选择2016年12月—2018年12月收治的188例FD,根据治疗方法的不同,分为对照组与观察组,每组各94例。对照组予以莫沙必利治疗,观察组予莫沙必利联合氟哌噻吨美利曲辛治疗,均治疗2周。比较治疗后临床疗效,观察治疗前、治疗后近端胃容积、胃底气体评分、胃排空率及血清胃肠激素[胃动素(motilin,MTL)、P物质(substance P,SP)、血管活性肠肽(vasoactive intestinal peptide,VIP)]变化情况。结果观察组治疗总有效率显著高于对照组,差异有统计学意义(P<0.01)。观察组治疗后饮用纯净水和显影剂后60 min时近端胃容积较对照组缩小,差异有统计学意义(P<0.01);与本组治疗前同一时间点比较,两组治疗后饮用纯净水和显影剂后30、60 min时近端胃容积缩小,差异有统计学意义(P<0.01)。与对照组比较,观察组治疗后胃底气体评分及VIP水平下降,胃排空率及MTL、SP水平升高,差异有统计学意义(P<0.01);与本组治疗前比较,两组治疗后胃底气体评分及VIP水平显著下降,胃排空率及MTL、SP水平明显升高,差异有统计学意义(P<0.01)。两组不良反应总发生率比较差异无统计学意义(P=0.30)。结论莫沙必利联合氟哌噻吨美利曲辛治疗可提高FD患者临床疗效,改善近端胃容积及胃排空功能,同时可调节血清胃肠激素水平。Objective To analyze the effects of mosapride combined with flupentixol and melitracen on proximal gastric diastole and gastric emptying function in patients with functional dyspepsia(FD).Methods A total of 188 patients with FD who were treated from December 2016 to December 2018 were selected.The patients were divided into control group(n=94)and observation group(n=94)according to the different treatment methods.The control group was given mosapride while the observation group was given mosapride combined with flupentixol and melitracen,and they were treated for 2 weeks.The clinical efficacy after treatment was compared between the two groups,and the proximal gastric volume,gastric fundus gas score,gastric emptying rate and levels of serum gastrointestinal hormones[motilin(MTL),substance P(SP),vasoactive intestinal peptide(VIP)]were observed before and after treatment.Results The total effective rate of treatment in observation group was significantly higher than that in control group(P<0.01).The proximal gastric volume at 60 min after drinking purified water and eikonogen in observation group was significantly reduced compared with that in control group(P<0.01).Compared with that in the same group at the same time point before treatment,the proximal gastric volume in the two groups was reduced at 30 and 60 min after drinking purified water and eikonogen(P<0.05).Compared with control group,the gastric fundus gas score and VIP level were decreased while the gastric emptying rate and levels of MTL and SP were increased after treatment in observation group(P<0.01).Compared with the same group before treatment,the gastric fundus gas score and VIP level after treatment in the two groups were decreased significantly while the gastric emptying rate and levels of MTL and SP were increased significantly(P<0.01).There was no significant difference in the total incidence rate of adverse reactions between the two groups(P=0.30).Conclusion In the treatment of FD patients,mosapride combined with flupentixol and melitracen can
