摘 要:目的探讨血小板淋巴细胞比值(PLR)预测乳腺癌术后复发、预后的价值.方法回顾性分析2012年3月至2014年3月山西省肿瘤医院收治的209例原发性乳腺癌患者的临床资料,计算其术前PLR,采用工作特征ROC曲线下面积(AUG)、Kaplan-Meier、Log-rank检验,分析术前PLR与乳腺癌术后复发转移、患者生存情况的相关性.结果209例乳腺癌患者随访过程中死亡24例,随访时间42(5~60)个月,PLR中位数为113.58.术前PLR预测乳腺癌复发转移AUG=0.628(95%CI 0.518~0.738),P=0.035,预测乳腺癌复发转移的临界值为125.48,灵敏度为61.5%,特异度为60.1%.术前PLR水平评估乳腺癌患者术后生存AUG=0.619(95%CI 0.504~0.734),P=0.058.淋巴结转移患者与无淋巴结患者总生存情况比较差异有统计学意义(χ2=4.858,P=0.028).结论术前PLR水平可作为预测乳腺癌术后复发转移的指标,但不能作为预测乳腺癌患者术后预后的有效指标.Objective To investigate the value of platelet lymphocyte ratio(PLR)in predicting postoperative recurrence and prognosis of breast cancer.Methods The clinical data of 209 patients with primary breast cancer admitted to Shanxi Provincial Cancer Hospital from March 2012 to March 2014 were retrospectively analyzed.The preoperative PLR was calculated.The area under the receiver operating characteristic(ROC)curve(AUG),Kaplan-Meier and Log-rank test were used to analyze the correlation between preoperative PLR and prognosis,postoperative recurrence and metastasis of breast cancer.Results Of the 209 breast cancer patients,24 died during follow-up.The follow-up time was 42(5-60)months,and the median PLR was 113.58.AUG of preoperative PLR in predicting recurrence and metastasis of breast cancer was 0.628(95%CI=0.518-0.738,P=0.035).and the critical value,sensitivity and specificity of predicting recurrence and metastasis of breast cancer were 125.48,61.5%and 60.1%respectively.Preoperative PLR levels were evaluated for postoperative survival of breast cancer patients with AUG=0.619(95%CI 0.504-0.734),P=0.058.There was a statistically significant difference in the overall survival between patients with lymph node metastasis and those without lymph node(χ2=4.858,P=0.028).Conclusions Preoperative PLR level can be used as an index to predict postoperative recurrence and metastasis of breast cancer,but it can not be used as an effective predictor of postoperative prognosis in breast cancer patients.
