剥脱性食管炎的影像诊断

摘　　要：目的探讨剥脱性食管炎的影像表现.方法回顾性分析27例经胃镜及临床随访证实的剥脱性食管炎患者的临床及影像资料.结果4例病变位于食管颈段至胸上段,21例病变位于食管胸上段,2例病变位于食管胸上段至中段,所有病例均未累及食管下段.钡餐透视显示:13例表现为食管腔内囊袋状影,其内可见液-钡平面;5例表现为食管腔内圆形或椭圆形充盈缺损,4例为多发;7例表现为食管"双管征";2例表现为食管局部黏膜紊乱,见杂乱恒定小钡斑.所有病例钡餐透视动态观察均显示食管管壁柔软,对比剂通过顺利,未见梗阻征象.结论剥脱性食管炎钡餐透视X线表现为食管腔内囊袋状影、腔内圆形或椭圆形充盈缺损、双管征、杂乱恒定小钡斑,结合临床病史可明确该病的诊断.Objective To study the imaging findings of exfoliative esophagitis.Methods The clinical and imaging data of 27 patients with exfoliative esophagitis confirmed by gastroscope and clinical follow-up was analyzed retrospectively.Results There were 4 cases with the lesions locating in between the cervical and upper thoracic esophagus,21 cases with the lesions locating in the upper thoracic esophagus,2 cases with the lesions locating in between the upper thoracic and middle segment of esophagus.None of the lesions involved the lower esophagus.In barium meal fluoroscopy,13 cases showed intraluminal sac shadow with fluid-barium level;5 cases showed round or ellipse intraluminal filling defect,4 of the 5 cases were multiple lesions;7 cases showed double duct sign in the esophagus;2 cases showed disorders of local mucosa and random constant barium spot.Among all the patients,the esophagus wall was soft,and contrast medium went through smoothly without obstruction.Conclusion Exfoliative esophagitis through barium meal fluoroscopy can be showed as intraluminal sac shadow,round or ellipse intraluminal filling defect,double duct sign,random consatnt barium spot.All these symptoms combined with clinical history to make a definite diagnosis.
