乳腺癌增强MRI形态学特征及表观扩散系数值与相关分子生物学标记物的关系分析

摘　　要：目的探讨乳腺癌(BC)增强MRI形态学特征及ADC值与相关分子生物学标记物的关系.方法选取46例BC患者为研究对象,均行增强MRI检查和DWI检查,免疫组化染色法检测患者分子生物学标记物[雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体2(HER-2)、Ki-67]阳性表达情况,并对BC病变组织增强MRI形态学特征及ADC值与相关分子生物学标记物进行相关性分析.结果增强MRI诊断病变形态学的灵敏度94.29%、特异度72.73%、准确度89.13%,病变增强MRI形态学特征中的毛刺状边缘与ER阳性表达呈正相关(r=0.409,P<0.05),与PR、HER-2、Ki-67阳性表达无明显相关性(P>0.05);病变增强MRI形态学特征中的不均匀强化与HER-2、Ki-67阳性表达呈正相关(r=0.461、0.268,P<0.05),但与ER、PR阳性表达无明显相关性(P>0.05),病变ADC值与Ki-67阳性表达呈明显正相关(r=0.345,P<0.05),但与ER、PR、HER-2无明显相关性(P>0.05).结论增强MRI形态学特征在BC肿块形态学中有较高的诊断效能,其形态学特征、ADC值与ER、HER-2、Ki-67阳性表达密切相关.Objective To investigate morphological features of breast cancer(BC)enhanced MRI,ADC values and their relationships with related molecular biomarkers.Methods 46 patients with BC admitted to the were enrolled in the study.All patients underwent enhanced MRI scan and DWI examination.Immunohistochemical staining was performed to detect positive expressions of molecular biomarkers[estrogen receptor(ER),progesterone receptor(PR),human epidermal growth factor receptor-2(HER-2),Ki-67],The correlation analysis was performed between morphological features of enhanced MRI,ADC values and related molecules biomarkers in patients with BC.Results The sensitivity,specificity and accuracy for diagnosis of lesion morphology by enhanced MRI were 94.29%,72.73% and 89.13%,respectively.The burr-like margin in morphological features of enhanced MRI was positively correlated with positive expression of ER(r=0.409,P<0.05).which was not significantly correlated with positive expressions of PR,HER-2 or Ki-67(P>0.05).In morphological features of enhanced MRI,uneven enhancement was positively correlated with positive expressions of HER-2 and Ki-67(r=0.461,0.268,P<0.05).however,not significantly correlated with the positive expressions of ER and PR(P>0.05).The lesion ADC value was positively correlated with positive expression of Ki-67(r=0.345,P<0.05),not significantly correlated with ER,PR or HER-2(P>0.05).Conclusion The diagnostic efficiency of morphological features of enhanced MRI is high in the morphology of BC mass.Its morphological features and ADC value are closely related to positive expressions of ER,HER-2 and Ki-67.
