摘 要:目的利用MR DTI技术分析先天性耳聋患儿听觉传导通路及语言中枢微观结构的改变.方法对52例0~6岁先天性耳聋患儿及29例听力正常的儿童在3.0T MR进行颅脑DTI检查,采用基于ROI分析方法测量所有受试者双侧外侧丘系,下丘,内侧膝状体,听辐射,BA41、BA42、BA22、BA44、BA45区及BA39区FA值、λ‖值、λ⊥值及平均扩散率(MD)值,同时运用基于体素分析(VBA)及基于区域空间统计(TBSS)分析方法观察先天性感音神经性耳聋(SNHL)患儿全脑DTI的改变.结果(1)先天性耳聋患儿听觉传导通路及语言中枢微观结构的改变主要集中于下丘、BA22区及BA44区.(2)先天性耳聋患儿全脑结构的改变不是仅局限于或集中于听觉语言相关脑区,而且可伴有额叶、顶叶及枕叶等广泛区域的改变.结论DTI能够检测到常规MRI不能发现的细微脑白质损害,并可定量分析先天性耳聋患儿听觉传导通路及语言中枢微观结构的改变.Objective To analyze the changes of microstructure in the auditory pathway and speech center in children with congenital deafness by using MR DTI.Methods 52 children between 0 and 6 years with congenital deafness and 29 normal hearing subjects received 3.0T MR brain DTI.Four DTI parameters:FA value,axial diffusivity(λ‖)value,radial diffusivity(λ⊥)value and mean diffusivity(MD)value in the lateral lemniscus,inferior colliculus,medial geniculate body,auditory radiations,brodmann 41,42,22,44,45 and 39 areas were measured bilaterally with region of interest.Meanwhile,voxel-based analysis(VBA)and tract-based spatial statistics(TBSS)were used to analyze the changes of DTI in whole brain of children with congenital sensorineural hearing loss(SNHL).Results(1)The changes of auditory pathway and speech center in children with congenital deafness mainly concentrated in the inferior colliculus,BA22 and BA44 regions.(2)Congenital deafness in children with brain structure change is not only focus on auditory and language related brain regions,but also associate with a wide areas change,such as the frontal,parietal and occipital lobes.Conclusion DTI can detect subtle white matter lesions which is not detected by conventional MRI,and quantitatively analyze the changes of auditory pathway and speech center in children with congenital deafness.
