慢性萎缩性胃炎胃镜像与藏医四诊表现的相关性研究

摘　　要：目的:探索慢性萎缩性胃炎胃镜像与藏医四诊表现的相关性,为其临床证型分类提供科学依据。方法:收集166例慢性萎缩性胃炎患者症状、脉象、舌诊、尿诊资料,采集胃镜病理变化信息建立胃镜像数据库,Logistic回归分析其相关性。结果:与胃镜像相关的症状有便秘、口苦、腹泻、饱胀、抑郁、疼痛、烧心、上腹部不适、健忘、嗳气,与胃镜像相关的脉诊有沉脉、空脉、洪脉、弱脉、紧脉、实脉、右寸、左寸、右根,与胃镜像相关的尿诊有泡沫唾液样、嗅味大、浮着物厚、泡沫消失快,与胃镜像相关的舌诊有舌色红。结论:根据胃镜像与四诊的相关性,CAG分为隆型、赤巴型、培根型、混合型、培根赤巴型、隆赤巴型、培根隆型等7种,为其诊疗规范提供了证据。Exploring the correlation between the clinical manifestations of four diagnostic information of chronic atrophic gastritis and the pathological changes of gastroscopic images,and to provide an objective and quantitative scientific basis for the classification of CAG patterns.Methods:166 patients with CAG were enrolled in four diagnostic approaches including signs and symptoms,pulse signs,tongue diagnosis and urinalysis information.Gastric images were detected simultaneously by gastroscopy,and established the database of four diagnostic information and pathological changes.By using the logistic regression to analyze the correlation between the macroscopic and microscopic features of the the gastric mucosa.Results:Symptoms:Symptoms related to gastroscopic images include constipation,bitter mouth,diarrhea,bloating,depression,pain,chest burn,upper abdominal discomfort,forgetfulness,belching;Pulse diagnosis:The pulse beating associated with gastroscopic images are sunk,hollow,expanded,weak,tight,hard,right index finger,left index finger and right middle finger;Urinary diagnosis:The urinary indicatirs are associated with gastroscopic images include saliva-like bubble,large smell,thick float,foam disappears quickly;tongue examination:the tongue associated with gastroscopic images are diagnosed with tongue red.Conclusion:the correlation between four diagnostic information and gastroscopic changes have shown that patterns of CAG in Tibetan medicine that can classified with wind,bile,phlegm,wind-phlegm,bile-wind,phlegm-bile and blended types.The research has provided a scientific basis for the diagnosis and treatment of CAG from Tibetan medical perspective.
