睾丸鞘膜高分化乳头状间皮瘤1例报告兼文献复习

摘　　要：目的探讨睾丸鞘膜高分化乳头状间皮瘤的临床表现、影像特点、病理特征及治疗方法,加深对此疾病的认识。方法回顾分析1例睾丸鞘膜高分化乳头状间皮瘤患者的临床资料。结果患者接受右侧睾丸、鞘膜、精索切除术。术中见睾丸呈灰白色,血运差,鞘膜壁层见多发菜花状赘生物。术后病理:一侧睾丸体积6.0 cm×3.0 cm×3.0 cm,切面灰黄,质中;附睾已切开,长8.0 cm,直径1.0 cm。睾丸表面见多个乳头状突起,直径0.4~1.0 cm。镜下见:睾丸鞘膜乳头状增生样的上皮样细胞,轻度异性。考虑睾丸鞘膜高分化乳头状间皮瘤。结论根治性睾丸切除加阴囊皮肤切除是睾丸鞘膜恶性间皮瘤的推荐治疗方法,必要时可放化疗。而睾丸鞘膜高分化乳头状间皮瘤目前尚无统一治疗标准。本例患者接受右侧睾丸根治术,术后需要进一步的随访才能评估预后。Objective To explore the clinical manifestations,imaging features,pathological features,and treatment of well-differentiated papillary mesothelioma of tunica vaginalis,and to deepen the understanding of this disease.Methods The clinical data of a patient with well-differentiated papillary mesothelioma of tunica vaginalis were retrospectively analyzed.Results The patient underwent resection of the right testicular,sheath,and spermatic cord.During the operation,we could saw that the testes looked grey,with poor blood flow and multiple cauliflower vegetations in tunica vaginalis.Postoperative pathology showed that the testis was the size of 6.0 cm×3.0 cm×3.0 cm;the cut surface was yellowish;the epididymis was 8.0 cm in length and 1.0 cm in diameter.There were multiple papillary protrusions on the surface of the testis,with a diameter of 0.4-1.0 cm.In microscopic observation,mildly heterosexual papillary hyperplasia-like epithelial cells were displayed on tunica vaginalis,and it was diagnosed as well-differentiated papillary mesothelioma of tunica vaginalis.Conclusions Radical orchiectomy plus scrotal skin resection is the recommended treatment for malignant tunica vaginalis mesothelioma.Chemotherapy may be performed if necessary.There is no definite treatment for well-differentiated papillary mesothelioma in testis.This patient underwent radical right testicular surgery,and further follow-up was needed to assess the prognosis.
