摘 要:目的:探讨p16、Ki-67、E2F-1在皖北地区宫颈上皮内病变及宫颈癌组织中的表达情况,明确其中宫颈癌进展中的表达趋势,为宫颈癌及癌前病变的早期临床诊疗及预后判断提供可靠依据。方法:收集蚌埠医学院第一附属医院临床病理科2016至2017年宫颈癌组织、宫颈高级别上皮内病变组织、宫颈低级别上皮内病变及对照组宫颈慢性炎组织各50例,免疫组化检测p16、Ki-67及E2F-1在宫颈组织中的表达情况。结果:p16、Ki-67及E2F-1在对照组宫颈慢性炎、低级别宫颈上皮内病变、高级别宫颈上皮内病变及宫颈癌中表达的阳性率分别为4.0%、28.0%、80.0%、92.0%;8.0%、34.0%、76.0%、94.0%;10.0%、32.0%、60.0%、80.0%,阳性率表达存在差异性(χ^2=105.2,P<0.05;χ^2=92.3,P<0.05;χ^2=52.2,P<0.05)。组间分析p16、Ki-67、E2F-1阴性对照组与各实验组之间存在显著性差异,低级别组与高级别组及宫颈癌组间存在显著性差异,高级别组与宫颈癌组间不存在统计学意义。相关性分析显示p16、Ki-67、E2F-1蛋白表达三者之间具有相关性。结论:子宫颈癌的逐级形成与p16、Ki-67、E2F-1蛋白过表达呈正相关关系,p16、Ki-67联合E2F-1检测可作为早期诊断及筛查宫颈上皮内病变及宫颈癌的标志物,为临床诊断及治疗供有效指导。Objective:To investigate the expression of p16,Ki-67 and E2F-1 in cervical intraepithelial lesions and cervical carcinoma and clarify the expression trend of cervical carcinoma progression,so as to provide a reliable basis for clinical early diagnosis and prognosis of cervical cancer and precancerous lesions.Methods:The tissues of cervical carcinoma,the high-grade intraepithelial lesions of the cervix and the low-grade intraepithelial lesions of the cervix and the chronic inflammatory as the control groups of 2016-2017 were collected from the Department of Pathology of First Affiliated Hospital of Bengbu Medical College.Immunohistochemistry method was used to determinate the expression of p16,Ki-67 and E2F-1 in cervical tissues.Results:The positive expression rates of p16,Ki-67 and E2F-1 in control group,low-grade cervical intraepithelial lesion group,high-grade cervical intraepithelial lesion group and cervical carcinoma group were 4.0%,28.0%,80.0%,92.0%;8.0%,34.0%,76.0%,94.0%and 10.0%,32.0%,60.0%,80.0%,respectively.There were significant differences among the three groups(χ^2=105.2,P<0.05;χ^2=92.3,P<0.05;χ^2=52.2,P<0.05).There were significant differences between the control groups and the experimental groups.There were significant differences between the low-grade group and the high-grade group and the cervical carcinoma group.There was no statistical difference between the high-grade group and the cervical carcinoma group.The correlation analysis showed that there was correlation between p16,Ki-67,E2F-1 protein expression and lesion grade.Conclusion:The progressive formation of cervical carcinoma was positively correlated with the over expression of p16,Ki-67 and E2F-1.p16,Ki-67 combined with E2F-1detection could be used as a marker for early diagnosis and screening of cervical intraepithelial lesions and cervical carcinoma,which could provide effective guidance for clinical diagnosis and treatment.
