摘  要:目的探讨糖原合成酶激酶-3(GSK-3β)、基质细胞衍生因子-1(SDF-1)、正五聚体蛋白(PTX3)与急性脑梗死患者早期神经功能恶化END的关系。方法回顾性分析北京老年医院2006年1月至2018年1月间收治的急性脑梗死患者120例,其中发生END患者58例(END组),未出现END患者(对照组)62例,比较两组患者临床基线资料以及血脂GSK-3β、SDF-1、PTX3水平。结果两组患者性别、年龄、高血压、糖尿病、入院时美国国立卫生研究院卒中量表(NIHSS)评分及吸烟人数比较,差异无统计学意义(P>0.05)。END组GSK-3β水平(3.98±0.23)μg/L比对照组(2.18±0.13)μg/L升高,差异有统计学意义(t=53.21,P<0.05);END组SDF-1水平(1107.03±121.24)pg/mL比对照组(2127.33±147.38)pg/mL降低,差异有统计学意义(t=41.26,P<0.05);END组PTX3水平(22.05±2.42)pg/mL比对照组(13.05±2.20)pg/mL升高,差异有统计学意义(t=21.34,P<0.05)。Logistic分析显示,GSK-3β(OR=4.455,95%CI:2.489~7.973,P<0.01),SDF-1(OR=0.283,95%CI:0.131~0.612,P<0.01),PTX3(OR=2.962,95%CI:1.265~6.935,P<0.05)与END发生有关。结论SDF-1水平降低和GSK-3β、PTX3水平增高是急性脑梗死发生END的影响因素,可能对临床评估END具有指导意义。Objective To study the relationship between glycogen synthase kinase-3(gsk-3β),stromal cell-derived factor-1(SDF-1),pentraxin 3(PTX3)and early neurological deterioration(END)in acute cerebral infarction.Methods 120 patients with cerebral infarction were included,they were divided into END group(58 cases)and control group(62 cases).The baseline clinical data,serum levels of GSK-3β,SDF-1 and PTX3 were compared between the two groups.Results There were no differences in gender,age,hypertension,diabetes,NIHSS score and proportion of smoking history between END group and control group(P>0.05).The level of GSK-3βin END group[(3.98±0.23)μg/L]was higher than that in the control group[(2.18±0.13)μg/L](t=53.21,P<0.05).The level of SDF-1 in END group[(1107.03±121.24)pg/mL]was higher than that in the control group[(2127.33±147.38)pg/mL](t=41.26,P<0.05).The level of PTX3 in END group[(22.05±2.42)pg/Ml]was higher than that in the control group[(13.05±2.20)pg/mL](t=21.34,P<0.05).Logistic analysis showed the levels of GSK-3β(OR=4.455,95%CI:2.489~7.973,P<0.01),SDF-1(OR=0.283,95%CI:0.131~0.612,P<0.01),PTX3(OR=2.962,95%CI:1.265~6.935,P<0.05)were related with END.Conclusion Elevated GSK-3β,PTX3 levels and reduced SDF-1 level are the independent influencing factors for END in acute cerebral infarction,and play a certain role in clinical assessment of END.
