彩色多普勒超声对小乳腺癌与乳腺增生诊断及鉴别诊断的评价

摘　　要：目的探讨彩色多普勒超声在小乳腺癌与乳腺增生诊断与鉴别诊断中的应用价值。方法选取经手术和病理诊断为小乳腺癌65例(观察组)和乳腺增生患者65例(对照组),均经病理学确诊。观察对比2组患者的超声影像、血流动力学指标收缩期峰值流速(SPV)及阻力指数(RI)。结果观察组病灶边界不光滑、肿块不规则、回声不均匀比例均高于对照组(P<0.05);观察组SPV、RI亦高于对照组(P<0.05)。结论小乳腺癌与乳腺增生的彩色多普勒超声影像、血流动力学指标有一定的区别,彩色多普勒超声有助于两者的鉴别与诊断。Objective To evaluate the value of color Doppler ultrasonography in diagnosis and differential diagnosis of small breast cancer and mammary gland hyperplasia.Methods Sixty-five patients with small breast cancer(observation group)and 65 patients with mammary gland hyperplasia(control group)were collected in this study.All cases were confirmed by surgical and pathological findings.Ultrasonic images,peak systolic velocity(SPV)and resistance index(RI)were observed in both groups.Results Compared with control group,the observation group showed a significant increase in proportions of lesions with rough boundaries,tumors with irregular shape and masses with uneven echoes,as well as in SPV and RI(P<0.05).Conclusion Color Doppler ultrasound images and hemodynamic indexes are distinctly different between small breast cancer and mammary gland hyperplasia.Therefore,color Doppler ultrasonography is helpful for the differential diagnosis of small breast cancer and mammary gland hyperplasia.
