摘　　要：目的探讨piwil2在口腔鳞状细胞癌患者中的表达及手术治疗对整形美容的影响。方法选择2010年1月-2017年1月口腔鳞状细胞癌患者56例作为对象,所有患者均拟行手术治疗,术中取病灶组织及癌旁组织(距离病灶≥3cm),术后对患者进行3年随访。采用免疫组织化学法测定不同组织中piwil2蛋白表达水平;查阅病理资料,记录不同性别、年龄、肿瘤直径、分化程度及肿瘤分期状态下piwil2阳性率。所有患者术后均常规进行口腔整形美容,并借助同意问卷对患者美容前、后整体美观、口腔形态、面部外貌、口腔咀嚼功能及吞咽功能进行调查,记录患者生存期,比较两组piwil2表达水平、美观、生存期。结果所有患者均顺利完成手术,术后1例患者发生感染,患者球愈合良好,未见明显愈合不良,获得良好的美观。口腔鳞状细胞癌患者病灶组织中piwil2蛋白阳性率高于癌旁组织中(P<0.05);免疫组化结果表明:piwil2蛋白在口腔鳞状细胞癌患者病灶组织中阳性率较高以深、棕黄色为主,而在癌旁组织中黄色面积相对较小;口腔鳞状细胞癌患者中piwil2蛋白阳性率与性别、年龄、肿瘤部位无统计学意义(P>0.05);与肿瘤直径、TNM分期及分化类型具有统计学意义(P<0.05);口腔鳞状细胞癌患者手术后整体美观、口腔形态、面部外貌、口腔咀嚼功能及吞咽功能评分均高于手术前(P<0.05)。结论piwil2在口腔鳞状细胞癌患者中呈高表达,其阳性率能反映患者疾病严重程度,患者手术后加强其整形美容可获得良好的口腔美观,值得推广应用。Objective To investigate the expression of PIWIL2 in patients with oral squamous cell carcinoma and the effect of surgical treatment on plastic and cosmetic surgery.Methods 56 patients with oral squamous cell carcinoma from January 2010to January 2017 were selected as the subjects.All patients were scheduled to undergo surgical treatment.During the operation,the focus tissue and paracancerous tissue(distance from the focus≥3cm)were taken and the patients were followed up for 3 years.The expression level of PIWIL2 in different tissues was measured by immunohistochemistry,and the pathological data were consulted to record the positive rate of PIWIL2 in different sex,age,tumor diameter,differentiation degree and tumor stage.All patients underwent oral plastic surgery and Cosmetology,and investigated the overall beauty,oral morphology,facial appearance,oral chewing function and swallowing function of patients before and after cosmetology with the help of consent questionnaire,recorded the survival period of patients,and compared the expression level,beauty and survival period of PIWIL2 in the two groups.Results All the patients successfully completed the operation.One patient developed infection after the operation.The ball of the patient healed well,no obvious poor healing was found,and good appearance was obtained.The positive rate of PIWIL2 protein in the lesions of OSCC was higher than that in the paracancerous tissues(P﹤0.05);the results of immunohistochemistry showed that the positive rate of PIWIL2 protein in the lesions of OSCC was mainly dark and brown yellow,while the yellow area in the paracancerous tissues was relatively small;the positive rate of PIWIL2 protein in OSCC was higher than that in the sex,age,age,age,and so on There was no significant difference in tumor location(P﹥0.05),tumor diameter,TNM stage and differentiation type(P﹤0.05),and the scores of overall beauty,oral morphology,facial appearance,oral chewing function and swallowing function of patients with OSCC after operation were hig
