摘  要:目的探讨激活的激酶C受体1(RACK1)、热休克转录因子1(HSF1)、E-钙黏蛋白(E-cadherin)在宫颈癌中的表达及其与临床特征、预后的关系。方法选取68例宫颈癌患者、28例宫颈上皮内瘤变(CIN)患者、23例因子宫肌瘤行全子宫切除术患者的宫颈癌组织、CIN组织及正常宫颈组织。比较各组RACK1、HSF1、E-cadherin阳性表达率,并分析RACK1、HSF1、E-cadherin阳性表达与宫颈癌临床特征的关系,及其对预后的影响。结果宫颈癌组织中RACK1、HSF1的阳性表达率均高于CIN组织及正常宫颈组织,E-cadherin的阳性表达率分别低于CIN组织及正常宫颈组织,差异均有统计学意义(P<0.05)。临床分期为Ⅲ期、深肌层浸润、低分化、有淋巴结转移的宫颈癌患者的宫颈癌组织中RACK1、HSF1阳性表达率分别高于临床分期为Ⅰ~Ⅱ期、浅肌层浸润、中~高分化、无淋巴结转移宫颈癌患者的宫颈癌组织,E-cadherin阳性表达率分别低于临床分期为Ⅰ~Ⅱ期、浅肌层浸润、中~高分化、无淋巴结转移宫颈癌患者的宫颈癌组织,差异均有统计学意义(P<0.05)。多因素Cox比例风险模型显示,年龄>50岁、腺癌、临床分期为Ⅲ期、深肌层浸润、低分化、有淋巴结转移、RACK1阳性表达、HSF1阳性表达均是影响宫颈癌预后的危险因素,E-cadherin阳性表达是宫颈癌预后的保护因素。结论RACK1、HSF1、E-cadherin可作为宫颈癌患者预后的参考指标。Objective To analyze the expression of receptor for activated C kinase 1(RACK1),heat shock transcription factor 1(HSF1)and E-cadherin in cervical cancer and the correlation with clinical features and prognosis.Method The analysis included 68 patients with cervical cancer,28 cases with cervical intraepithelial neoplasia(CIN),23 patients of hysteromyoma who underwent total hysterectomy,cervical cancer tissues,CIN tissues and normal cervix tissues were collected from these patients,respectively.Positive expression rates of RACK1,HSF1 and E-cadherin of each group were compared,the relationship between RACK1,HSF1,E-cadherin expression and the clinical characteristics of cervical cancer was analyzed,and the prognostic influence was evaluated.Result Positive expression rates of RACK1 and HSF1 in cervical cancer tissues were higher than CIN and normal cervix tissues,while the positive expression rate of E-cadherin in cervical cancer tissues was lower compared to CIN tissues and normal cervix tissues,with statistically significant differences observed(P<0.05).The positive expression of RACK1 and HSF1 in cervical cancer tissues with clinical stageⅢ,deep myometrial infiltration,low differentiation,and positive lymph node metastasis were significantly higher compared to those with clinical stageⅠ-Ⅱ,superficial myometrial infiltration,moderate to high differentiation,and negative lymph node metastasis,besides,positive expression rate of E-cadherin was lower in cervical cancer tissues with clinical stageⅠ-Ⅱ,superficial myometrial infiltration,moderate to high differentiation and negative lymph node metastasis,indicating statistically significant differences(P<0.05).Multivariate Cox proportional hazard regression revealed that,age>50,adenocarcinoma,clinical stage□,deep myometrial infiltration,low differentiation,lymph node metastasis,positive RACK1 expression and positive HSF1 expression were all risk factors affecting the prognosis of cervical cancer,in contrast,positive E-cadherin expression was a protective one.C
