十味芪黄益肾方对慢性肾衰竭脾肾亏虚夹瘀浊证患者氧化应激指标的干预作用

摘　　要：目的:观察十味芪黄益肾方对慢性肾衰竭(CRF)脾肾亏虚夹瘀浊证患者氧化应激的影响。方法:随机将80例患者纳入对照组和中药组,每组40例,实际完成78例(对照组38例,中药组40例)。两组均给予中药保留灌肠+西医基础治疗,中药组加用十味芪黄益肾方口服,疗程均为8周。观察两组临床疗效和中医证候疗效,检测两组病例治疗前后血清中活性氧(ROS)、丙二醛(MDA)、晚期氧化蛋白产物(AOPP)、晚期糖基化终产物(AGEs)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)、胱抑素C(CysC)水平及尿总蛋白/肌酐(TPCR),计算肾小球滤过率(eGFR)。结果:中药组临床疾病治疗总有效率及中医证候总有效率均优于对照组(P<0.05);中药组ROS、MDA、AOPP、AGEs水平较本组治疗前显著下降,且下降水平明显优于对照组(P<0.01);中药组SOD、GSH-px水平较本组治疗前有显著升高,且升高水平明显优于对照组(P<0.01);中药组TPCR、CysC水平较本组治疗前明显下降(P<0.01),且优于同期对照组(P<0.05);中药组eGFR水平较本组治疗前明显升高(P<0.01),且优于同期对照组(P<0.05)。结论:西医治疗联合灌肠的基础上加服十味芪黄益肾方能显著改善CRF脾肾亏虚夹瘀浊证患者的临床症状、中医证候和生化指标,降低患者ROS、MDA、AOPP、AGEs、TPCR、CysC水平,升高SOD、GSH-px、eGFR水平,其机制可能与抑制体内氧化应激状态,提高机体抗氧化能力,从而延缓肾间质纤维化进程有关。Objective:To observe the effect of Shiwei Qihuang Yishen Decoction on the oxidative stress index of patients with chronic renal failure with spleen and kidney deficiency with blood stasis and turbidity syndrome.Methods:80 patients were divided into control group and Chinese medicine group according to the random number table method.40 cases in each group,78 cases were finally completed.including 38 cases in the control group and 40 cases in the Chinese medicine group.Two groups were given the basic treatment of western medicine and retention enema of TCM.The Chinese medicine group was added Shiwei Qihuang Yishen Decoction in oral for eight weeks.To observe the total effective rates on disease and on traditional Chinese medicine syndrome of the two groups.Serum ROS,MDA,AOPP,AGEs,SOD,GSH-Px,CysC levels and urinary TPCR were measured before and after treatment in both groups,and glomerular filtration rate was calculated.Results:The total effective rate in the Chinese medicine group was 82.50%,and the total effective rate of TCM syndrome was 85.00%,which was superior to these of the control group by 52.64% and 47.36%,respectively(P<0.05).After treatment,the levels of ROS,MDA,AOPP and AGEs in the Chinese medicine group were significantly lower than those before the treatment,and its effects were significantly better than these in the control group(P<0.01).The levels of SOD and GSH-px were significantly higher than those before the treatment,and significantly better than these of the control group(P<0.01),TPCR and CysC levels decreased significantly(P<0.01),and were superior to these of the control group(P<0.05),and eGFR levels were significantly increased(P<0.01),which was better than these of the control group(P<0.05).Conclusion:Shiwei Qihuang Yishen Decoction can improve the clinical symptoms,TCM syndromes and renal function indicators of patients with chronic renal failure with spleen and kidney deficiency with blood stasis and turbidity syndrome,reduce ROS,MDA,AOPP,AGEs,TPCR,CysC levels,increase SOD,GSH-px,eGFR le
