儿童脓毒症患者低白蛋白水平下的营养及免疫状况分析

摘　　要：目的探讨儿童脓毒症患者低白蛋白水平时的营养及免疫状况。方法采用回顾性研究方法,收集164例符合脓毒症诊断标准的患儿病例资料,进行统计分析。结果164例儿童脓毒症患者,出院30天存活率为51.8%,病原微生物检测阳性主要是细菌(96例,52.7%)和病毒(49例,26.9%)。根据白蛋白水平,分为4组。与正常对照组相比,轻度、中度、重度低白蛋白组的H-CRP水平逐渐升高,红细胞、淋巴细胞水平逐渐降低,差异有统计学意义(P<0.05)。与正常对照组相比,轻度低白蛋白组的Ig G降低,重度低白蛋白组的IgA升高,中度低白蛋白组的IgM降低,差异有统计学意义(P<0.05)。结论白蛋白水平降低会加重脓毒症患儿的营养和免疫功能损伤。早期补充相应的营养物质,将有助于重建免疫系统,提高儿童脓毒症的治愈率。OBJECTIVE To investigate the nutrition and immune status of children with sepsis at low albumin levels.METHODS The data of 164 children with sepsis were collected and analyzed using retrospective study method.RESULTS Of all164 children with sepsis,the 30-day survival rate was 51.8%,and the main positive microorganisms were bacteria(96 cases,52.7%)and virus(49 cases,26.9%).According to the albumin level,it was divided into four groups.Compared with the normal control group,the level of H-CRP increased gradually,while the level of erythrocyte and lymphocyte decreased gradually in mild albumin group,moderate albumin group and severe low albumin group,with significant difference(P<0.05).Compared with the normal control group,the level of IgG decreased in mild albumin group,IgA increased in severe low albumin group,IgM decreased in moderate albumin group,with significant difference(P<0.05).CONCLUSION Lower albumin levels may aggravate nutritional and immune impairment in children with sepsis.Early supplementation of corresponding nutrients could help rebuild the immune system and improve the cure rate of sepsis in children.
