摘　　要：髓外硬膜下肿瘤是神经外科常见疾病之一,多为良性肿瘤,神经源性肿瘤与脊膜瘤在良性肿瘤中占比较高。目前,临床主要治疗方法为手术切除病灶,传统常规术式易破坏脊柱稳定性,对机体创伤性较大,患者术后恢复慢、并发症多。随着医疗技术的进步,医疗资源不断扩充,半椎板、锁孔等新的入路为手术方案提供了更多的选择,可更大程度保护脊柱稳定性,促进患者术后恢复。半椎板入路术用于髓外硬膜下肿瘤的治疗能够有效保护机体棘间、棘上韧带及相应的关节突,对背部椎体完整性与稳定性造成的损害小,且术后并发症少,成为临床治疗髓外硬膜下肿瘤的主要手段。Subdural extramedullary tumor is one of the common diseases in neurosurgery,most of which are benign tumors,and the neurogenic tumors and meningioma account for a higher proportion in the benign tumors.At present,the main clinical treatment method is surgical resection of the lesion,and the conventional surgical procedure is traumatic and susceptible to risk of destroying the stability of the spine,resulting in slow postoperative recovery and more complications.With the advancement of medical technology,the continuous expansion of the medical resources,and the development of new approaches such as semi-lamina and keyhole,more options are available to protect spinal stability to a greater extent and promote the postoperative recovery.The semi-lamina approach of surgery in the treatment of subdural extramedullary tumor can effectively protect the interspinous,supraspinous ligament and corresponding articular processes,which has little damage to the integrity and stability of the back vertebral body and less postoperative complications,therefore has become the main method in the clinical practice.
